Molecular epidemiology of bacteraemic vancomycin-resistant Enterococcus faecium isolates and in vitro activities of SC5005 and other comparators against these isolates collected from a medical centre in northern Taiwan, 2019-2020

Summary: This study investigated the molecular epidemiology of VREfm bacteremic isolates in northern Taiwan from 2019-2020 and evaluated their susceptibility to last-line antibiotics and a new antimicrobial agent, SC5005. The results showed that all VREfm isolates harbored the vanA gene, with the most prevalent sequence types being ST17 and ST78. Four new STs were identified, and the arrival of multidrug-resistant ST1885 was observed in Taiwan. SC5005 was effective against all VREfm isolates, including those non-susceptible to last-line antibiotics, without detectable resistance.

Summary: This study aimed to evaluate the effectiveness of the FilmArray blood culture identification (BCID) panel in detecting vanM-type vancomycin-resistant enterococci (VRE) clinical isolates. By using conventional PCR and sequencing, the van genotypes of twenty VRE strains were confirmed. All VRE strains were investigated using the FilmArray BCID panel and the results were compared with conventional methods. The FilmArray BCID panel was found to effectively detect vanM-type VRE by adjusting the expected melting temperature (Tm) range.

Summary: This study investigated the frequency of intestinal VRE carriage in intensive care patients in Beijing. The majority of VRE carriers were Enterococcus faecium, with 78.3% of the VRE strains being vanA positive. Factors associated with VRE carriage were patients aged >65 years and recent use of third-generation cephalosporins.

Summary: Enterococcus faecium has become a dominant species in vancomycin-resistant enterococci (VRE) infections in healthcare settings. Variants such as vanB ST796 and vanA ST1421 have emerged, originally from Australia, and caused VRE outbreaks in other countries. The detection of vancomycin variable enterococci (VVE) belonging to ST1421 in Europe, which exhibit a vancomycin-susceptible phenotype but can revert to resistance, is of concern. This study also explores the use of genome sequencing to understand the evolution and spread of VRE.

Interhospital transmission of vancomycin-resistant Enterococcus faecium in Aomori, Japan

Summary: This study investigated the outbreaks of vancomycin-resistant Enterococcus faecium (VREf) in six general hospitals in Aomori prefecture, Japan. The findings revealed that this was the first multi-jurisdictional outbreak of VREf sequence type 1421 in Japan. The study suggests that continuous monitoring of VRE detection in local medical regions and effective communication among hospitals are required to prevent VREf outbreaks.
