Summary: Intravenous thrombolytics and endovascular thrombectomy are two treatment methods for ischemic stroke. The best approach to combine these therapies for eligible patients is still uncertain. Randomized trials comparing direct thrombectomy approach with intravenous thrombolytics followed by endovascular thrombectomy have shown similar functional outcomes, with better reperfusion after thrombectomy. However, bridging thrombolytics may increase the risk of symptomatic hemorrhage.

Summary: This study investigated the effect of treatment delays on the risks and benefits of intravenous thrombolysis (IVT) before mechanical thrombectomy (MT). The results showed that overall and in-hospital treatment delays did not modify the effect of IVT on functional independence. However, exploratory analysis of secondary clinical outcomes suggested a potentially greater benefit of IVT with shorter in-hospital delays.

Time Since Stroke Onset, Quantitative Collateral Score, and Functional Outcome After Endovascular Treatment for Acute Ischemic Stroke

Summary: In patients with ischemic stroke, the time to CT angiography (CTA) and collateral status are important factors for prognosis. This study aimed to assess the relationship between time to CTA and a quantitatively determined collateral score and whether the collateral score modified the relationship between time to recanalization and functional outcome. The results showed that time to CTA did not affect the collateral status, and a shorter time to recanalization was associated with better functional outcome regardless of the baseline collateral status.
