Evolocumab's Long-Term Mortality Risk Unclear Due to Shortened Follow-Up of FOURIER

Summary: The FOURIER trial was terminated earlier than planned, with a higher event rate than expected, but only slightly higher numbers of key secondary events. The variety of events included non-atherosclerotic myocardial infarctions, hemorrhagic strokes, and cardiovascular deaths unrelated to heart attacks or strokes. Changes in sample size during the trial and a divergence in all-cause mortality rates after 2 years of follow-up were observed.

Patient-tailored antithrombotic therapy following percutaneous coronary intervention

Summary: Dual antiplatelet therapy is the standard of care for patients with chronic coronary syndrome and acute coronary syndrome undergoing percutaneous coronary intervention, but selecting the optimal duration and composition of treatment poses a major challenge. Current guidelines recommend risk stratification for tailoring treatment, yet various risk scoring methods and testing approaches have not been widely adopted in clinical practice. Further research and validation are needed to determine the effectiveness of these risk stratification methods in improving clinical outcomes.

Predictors, Type, and Impact of Bleeding on the Net Clinical Benefit of Long-Term Ticagrelor in Stable Patients With Prior Myocardial Infarction

Summary: In patients with prior myocardial infarction, using ticagrelor 60 mg twice daily predominantly leads to spontaneous gastrointestinal bleeding. A history of spontaneous bleeding requiring hospitalization or the presence of anemia can identify patients at higher risk of bleeding, and the absence of either predictor may indicate patients more likely to benefit from ticagrelor with a favorable net benefit.

Screening for Atrial Fibrillation in the Older Population A Randomized Clinical Trial

Summary: In this study, home-based AF screening with a wearable cECG monitor significantly increased AF detection and anticoagulant therapy initiation, especially in older community-dwelling individuals with hypertension. Intermittent oscillometric screening with a BP monitor was found to be less effective for detecting paroxysmal AF compared to continuous ECG monitoring. Further large trials are needed to evaluate the benefits and risks of AF screening with hard clinical outcomes.
