The Impact of Center Volume on Outcomes in Lung Transplantation

Zhizhou Yang et al.

Summary: This study analyzed clinical data of lung transplant patients and found an association between center volume and 1-year survival rate. The optimal threshold for volume was identified as 33 cases per year, as exceeding this number did not provide additional benefits.

Thrombosis and Bleeding in Extracorporeal Membrane Oxygenation (ECMO) Without Anticoagulation: A Systematic Review

Sven R. Olson et al.

Summary: The study reviewed thrombosis and bleeding outcomes in ECMO patients without continuous systemic anticoagulation. Results showed that while the incidence of bleeding and thrombosis was higher, the risk of circuity and patient thrombosis in anticoagulant-free ECMO patients was comparable to those receiving continuous systemic anticoagulation during ECMO.

Association of total pre-existing comorbidities with stroke risk: a large-scale community-based cohort study from China

Ya Zhang et al.

Summary: The study found that hypertension, congenital heart disease, previous stroke, and diabetes are four pre-existing comorbidities that are independently and positively associated with the risk for all types of stroke. Participants with 1 or more pre-existing comorbidities had nearly 2 and nearly 3 times higher risk of stroke, respectively, compared to those without pre-existing conditions.

Acute Brain Injury in Postcardiotomy Shock Treated With Venoarterial Extracorporeal Membrane Oxygenation

Christopher Wilcox et al.

Summary: This study found that acute brain injury is common in postcardiotomy VA-ECMO patients and is associated with worse outcomes. Patients with central cannulation were more likely to experience acute strokes, while intracardiac thrombus was significantly associated with acute brain injury.

Venous thromboembolism in lung transplant recipients real world experience from a high volume center

Summary: This study reviewed the incidence of VTE post lung transplantation and identified risk factors such as the use of cardiopulmonary bypass (CPB) and interruption of VTE prophylaxis. Measures to ensure consistent and uninterrupted prophylaxis may help decrease VTE incidence after lung transplantation.

OPTN/SRTR 2019 Annual Data Report: Lung

M. Valapour et al.

Summary: The number of lung transplants performed is increasing annually with decreasing waitlist mortality, attributed to more candidates listed for transplant and increasing number of donors. Despite these favorable trends, a small percentage of recovered lungs were not transplanted in 2019. Posttransplant survival rates have shown slight gains, but regional heterogeneity still exists.

The new allocation era and policy

Luke J. Benvenuto et al.

Summary: The Department of Health and Human Services issued the Final Rule in 1998 as guidance for organ transplantation and allocation policies, leading to two major changes in the lung allocation system. The first change introduced the lung allocation score (LAS) as the primary determinant for donor lung allocation. Although successful in prioritizing the sickest candidates, the LAS model did not address geographic disparities in lung supply and demand, leading to inequities in waiting list survival based on a patient's location.

Management of left ventricular thrombus: a narrative review

Summary: LVT is a serious complication of acute MI and non-ischemic cardiomyopathies. Predictors of LVT include anterior MI, involvement of LV apex, LV akinesis or dyskinesis, reduced LVEF, severe diastolic dysfunction, and large infarct size. Anticoagulant therapy for at least 3 months can reduce the risk of LVT-related events.

Ischemic and Hemorrhagic Stroke Among Critically Ill Patients With Coronavirus Disease 2019: An International Multicenter Coronavirus Disease 2019 Critical Care Consortium Study*

Sung-Min Cho et al.

Summary: In an international cohort of ICU patients with coronavirus disease 2019, stroke was infrequent. Hemorrhagic stroke, but not ischemic stroke, was associated with increased mortality. Further, both hemorrhagic stroke and ischemic stroke were associated with traditional vascular risk factors.

Risk Factors of Ischemic and Hemorrhagic Strokes During Venovenous Extracorporeal Membrane Oxygenation: Analysis of Data From the Extracorporeal Life Support Organization Registry

Sung-Min Cho et al.

Summary: This study found that approximately 5% of patients receiving venovenous extracorporeal membrane oxygenation experienced acute brain injury, with mortality rates doubling when ischemic or hemorrhagic strokes occurred. Risk factors such as lower pH, hypoxemia, and coagulation disturbances were associated with acute brain injury, highlighting the importance of further research on prevention and management strategies.
