Predictive value of the stress hyperglycemia ratio in patients with acute ST-segment elevation myocardial infarction: insights from a multi-center observational study

Wei Xu et al.

Summary: This study evaluated the predictive value of stress hyperglycemia ratio (SHR) in patients with acute ST-segment elevation myocardial infarction (STEMI) and found that SHR is independently associated with major cardiovascular adverse events (MACEs) and mortality risk. Furthermore, adding SHR to the Thrombolysis in Myocardial Infarction (TIMI) risk score significantly improved its predictive efficiency, especially in patients with diabetes mellitus (DM).

Summary: This study developed and validated a simple scoring system to predict the risk of post-AMI infection in patients undergoing PCI. The risk score showed good discrimination and calibration in both the development and validation cohort. It also demonstrated good performance for infection, in-hospital all-cause death, and MACE.

Prognostic value of stress hyperglycemia ratio on short- and long-term mortality after acute myocardial infarction

T. Schmitz et al.

Summary: This study investigated the association between stress hyperglycemia ratio (SHR) and admission blood glucose with short- and long-term mortality in diabetic and non-diabetic acute myocardial infarction (AMI) patients. The results showed a nonlinear association between higher admission glucose and SHR with higher 28-day mortality in both diabetic and non-diabetic patients. However, the association with long-term mortality was only significant in diabetic patients. These findings suggest that stress hyperglycemia plays a significant role in short-term prognosis for AMI patients, but less so for long-term prognosis.

Summary: The study identified U-shaped associations of SHR with MACCE and MACE rates at 2-year follow-ups, and J-shaped associations of SHR with in-hospital cardiac death and MI in ACS patients who underwent DES implantation. The inflection point of SHR for poor prognosis was found to be 0.78.

Stress-Induced Hyperglycaemia in Non-Diabetic Patients with Acute Coronary Syndrome: From Molecular Mechanisms to New Therapeutic Perspectives

Alessandro Bellis et al.

Summary: This study suggests that stress-induced hyperglycemia (SIH) at hospital admission for acute coronary syndrome is associated with poor outcome, especially in patients without known diabetes. Insulin treatment in these subjects was not correlated with the reduction of mortality, likely due to the fact that SIH represents an epiphenomenon of other pathological conditions in the context of an acute coronary syndrome.

Predictors and mid-term outcomes of nosocomial infection in ST-elevation myocardial infarction patients treated by primary angioplasty

Mariana Santos et al.

Summary: This study investigated the incidence, predictors, and prognosis of nosocomial infections in STEMI patients. Out of 1131 patients analyzed, 11.1% developed NI, mainly from respiratory and urinary tract. Age, peripheral arterial disease, infarct size were found to be related to both NI and 1-year adverse events. Smokers who developed NI had a higher 1-year risk of MACCE.

Summary: SIH is common in ACS patients and associated with adverse outcomes, but its exact definition is controversial; Insulin-based therapy's impact on ACS patient outcomes is inconsistent; The mechanism underlying the detrimental effects of SIH on ACS patients is unclear, with oxidative stress potentially playing a role.
