Quantification of Early Diffuse Myocardial Fibrosis Through 7.0 T Cardiac Magnetic Resonance T1 Mapping in a Type 1 Diabetic Mellitus Mouse Model

Hongkai Zhang et al.

Summary: In this study, early DMIF changes in a T1DM mouse model were evaluated using MRI T1 mapping, with ECV identified as an accurate imaging marker for quantitatively assessing DMIF changes over time. The findings suggest that ECV has the potential to accurately detect DMIF in the early stage, making it a useful imaging tool for assessing the need for early intervention in T1DM mice.

Right ventricular failure in left heart disease: from pathophysiology to clinical manifestations and prognosis

Frank L. Dini et al.

Summary: Right heart failure is a clinical syndrome characterized by symptoms and signs caused by dysfunction and/or overload of the right heart structures, mainly the right ventricle, resulting in systemic venous hypertension, peripheral edema, and decreased tissue perfusion ability of the right heart. RHF can be caused by various cardiac diseases, including coronary artery disease, hypertension, valvular heart disease, cardiomyopathies, and myocarditis. Early recognition and identification of the underlying cause of RHF are crucial for effective treatment and prognosis improvement.

Heart Failure: An Underappreciated Complication of Diabetes. A Consensus Report of the American Diabetes Association

Rodica Pop-Busui et al.

Summary: Heart failure is a common complication of diabetes, even in the absence of other cardiovascular diseases. With the increasing prevalence of diabetes globally, particularly type 2 diabetes, the burden of heart failure on the healthcare system is expected to rise. This consensus report aims to provide clear guidance to practitioners on screening and diagnosing heart failure in individuals with diabetes.

Emerging Techniques in Cardiac Magnetic Resonance Imaging

Rui Guo et al.

Summary: Cardiac magnetic resonance plays a unique role in noninvasively evaluating patients with cardiovascular disease, especially in characterizing myocardial tissue composition. Despite challenges in the United States, its utilization has significantly grown in Europe and other regions, positioning it as an important imaging modality in the management of cardiovascular disease. Advances in cardiac MR techniques, including faster image acquisition and quantitative myocardial tissue characterization, have been critical to its growth.

Evaluation of right ventricular systolic and diastolic dysfunctions in patients with type 2 diabetes mellitus with poor glycemic control by layer specific global longitudinal strain and strain rate

Jun Huang et al.

Summary: The study found that RV layer specific GLS values were significantly lower in T2DM patients compared to normal controls, while GLSr-A values were significantly higher. HbA1c levels were negatively correlated with GLSEpi. ROC analysis demonstrated high diagnostic efficacy of RV layer specific GLS and GLSr-A in T2DM patients, with GLSr-A in RVFSW showing the best diagnostic value for evaluating RV diastolic function.

Cardiovascular outcomes trials: a paradigm shift in the current management of type 2 diabetes

Melanie J. Davies et al.

Summary: Cardiovascular disease is the leading cause of mortality in patients with type 2 diabetes. Data from cardiovascular outcomes trials show that certain glucose-lowering medications, such as SGLT2 inhibitors and GLP-1 receptor agonists, have cardioprotective effects and can reduce cardiovascular events and mortality risk, as well as the risks of heart failure hospitalization, renal disease progression, and all-cause mortality. International guidelines now prioritize the use of these medications in the management of type 2 diabetes.

Summary: The CORDIALLY study found that a significant proportion of patients with type 2 diabetes and cardiovascular disease/chronic kidney disease in CEE did not receive cardioprotective type 2 diabetes medication. This may be due to lack of affordable access, perceived safety, non-adherence to the latest treatment guidelines, and lack of collaboration between physicians.

Cardiac magnetic resonance T1 mapping for evaluating myocardial fibrosis in patients with type 2 diabetes mellitus: correlation with left ventricular longitudinal diastolic dysfunction

Xi Liu et al.

Summary: The objective of this study was to evaluate myocardial fibrosis using cardiac magnetic resonance (CMR) T1 mapping in type 2 diabetes mellitus (T2DM) patients and investigate the association between left ventricular (LV) subclinical myocardial dysfunction and myocardial fibrosis. The results showed that T1 mapping could detect abnormal myocardial fibrosis early in patients with T2DM, which can cause a decline in the LV diastolic function. The study also found an independent association between diabetes duration and extracellular volumes (ECVs).

Fat-Secreted Ceramides Regulate Vascular Redox State and Influence Outcomes in Patients With Cardiovascular Disease

Nadia Akawi et al.

Summary: This study demonstrates that ceramides derived from adipose tissue can modulate vascular redox state in obesity, directly impacting cardiac mortality in patients with advanced atherosclerosis. Exogenous ceramides can increase vascular oxidative stress and inflammation, leading to adverse cardiovascular outcomes. Treatment with liraglutide in obese patients can suppress ceramide levels, potentially reducing the risk of cardiac mortality.
