Clozapine Therapy and COVID-19: A Systematic Review of the Prevalence Rates, Health Outcomes, Hematological Markers, and Patient Perspectives

Grace Giles et al.

Summary: This review found no compelling evidence that the immune system of clozapine users puts them at risk of COVID-19 and further complications. However, evidence of drug-infection interactions highlights the importance of adhering to consensus guidelines on clozapine therapy during the pandemic. Longitudinal studies are needed to examine the longer-term effects of COVID-19 and vaccination in this vulnerable population.

Clozapine haematological monitoring for neutropenia: a global perspective

Ebenezer Oloyede et al.

Summary: There is considerable variation in guidelines for monitoring hematological function in patients treated with clozapine globally. Standardizing guidelines for hematological monitoring worldwide would be beneficial.

Relaxation of the criteria for entry to the UK Clozapine Central Non-Rechallenge Database: a modelling study

Ebenezer Oloyede et al.

Summary: This study investigated the implications of implementing the US FDA monitoring criteria on clozapine use in the UK. The results showed that implementing the revised monitoring criteria would significantly reduce clozapine discontinuation for hematological reasons and greatly improve the mental health outcomes of these patients.

An evaluation of the variation and underuse of clozapine in the United Kingdom

Eromona Whiskey et al.

Summary: The study revealed significant regional disparities in clozapine prescribing in different areas of England, with only about a third of potentially eligible patients receiving the drug. The rate of clozapine prescribing in the UK was lower than in several European countries, highlighting regional inequity in access to the most effective treatment for refractory schizophrenia.

No adverse events were observed in clozapine-treated patients on extended hematologic monitoring intervals during the coronavirus pandemic in four psychiatric centers in Japan

Summary: The temporary extension of monitoring interval for clozapine use in Japan during the COVID-19 pandemic did not result in adverse events for patients. Further verification of the results of these emergency measures worldwide is necessary as hematological monitoring intervals have been extended globally.
