Sex Differences and Similarities in Valvular Heart Disease

Jacqueline T. DesJardin et al.

Summary: As the global population ages, the burden of valvular heart disease has increased substantially, affecting a larger proportion of women. While rheumatic valve disease is declining in high-income countries, age-related degenerative causes are on the rise. Calcific aortic stenosis and degenerative mitral regurgitation significantly affect elderly women, especially those with comorbidities. Women with valvular heart disease have been underrepresented in important research studies, leading to delayed surgical referrals and poorer postoperative outcomes compared to men. Recent efforts to include women in research and clinical trials have provided valuable insights into sex-based differences in epidemiology, pathophysiology, diagnosis, treatment options, outcomes, and prognosis.

How to Decide Between a Bioprosthetic and Mechanical Valve

Tirone David

Summary: Matching heart valve substitute to patients of different age groups involves careful consideration. Mechanical valves are more durable but require lifelong anticoagulation, while bioprosthetic valves are suitable for older patients and have better durability.

Standardization of Prothrombin Time/International Normalized Ratio (PT/INR)

Summary: Prothrombin time (PT) and international normalized ratio (INR) play crucial roles in anticoagulant therapy, but variability in INR values between laboratories may still exist due to differences in reagents and coagulometer characteristics.

Characteristics, interventions and outcomes of patients with valvular heart disease hospitalised in China: a cross-sectional study

Xinghe Huang et al.

Summary: Little is known about contemporary characteristics and management of valvular heart disease (VHD) in China. This study provides insights into the clinical characteristics, etiology, types of VHD, interventions, and outcomes of VHD patients in China. The study reveals that degenerative origin is the predominant etiology among VHD patients, while rheumatic origin is most common in patients with moderate or severe VHD. Strategies targeting degenerative VHD and addressing undertreatment of severe VHD patients, especially those at high operative risk, should be promoted.

Massively parallel characterization of CYP2C9 variant enzyme activity and abundance

Clara J. Amorosi et al.

Summary: The CYP2C9 gene encodes an enzyme that plays a crucial role in metabolizing small molecule drugs, and its variants can impact drug response and increase the risk of bleeding. By utilizing the clickseq and VAMP-seq technologies, researchers measured the activity and cellular abundance of numerous CYP2C9 variants, revealing a significant portion with decreased activity and functional differences attributed to changes in protein abundance.

Current status and etiology of valvular heart disease in China: a population-based survey

Ying Yang et al.

Summary: The epidemiology of valvular heart disease (VHD) in China has been studied through a national survey, revealing a weighted prevalence of 3.8% and an estimated 25 million patients in the country. VHD prevalence increases with age, with higher rates in those with hypertension or chronic kidney disease. Rheumatic heart disease remains the major cause of VHD, but there is a significant increase in degenerative heart disease, with age and hypertension being important markers of VHD.

New Internet-Based Warfarin Anticoagulation Management Approach After Mechanical Heart Valve Replacement: Prospective, Multicenter, Randomized Controlled Trial

Zhihui Zhu et al.

Summary: The study demonstrated that internet-based warfarin management resulted in significantly higher TTR and fraction of time in the therapeutic range compared to conventional management, with lower frequency of bleeding and embolic events. Logistic regression analysis showed that internet-based management increased TTR and reduced the risk of bleeding and embolic events, while low TTR was identified as a risk factor for these events.
