Systolic and diastolic left ventricular function in children with primary hypertension: a systematic review and meta-analysis

Summary: Objective: This systematic review and meta-analysis evaluated left ventricular systolic and diastolic function in children and adolescents with primary hypertension. The results showed that hypertensive children and adolescents had increased heart rate and fractional shortening, but no difference in ejection fraction. They also exhibited lower E/A values, higher E/e' values, and greater global longitudinal stress compared to normotensive controls.

Isolated systolic hypertension is associated with increased left ventricular mass index and aortic stiffness in adolescents: a cardiac magnetic resonance study

Summary: The clinical significance of isolated systolic hypertension (ISH) among hypertensive adolescents is still undetermined. In this study, cardiac magnetic resonance was used to evaluate a group of suspected primary hypertension patients, and it was found that ISH patients had more pronounced markers of cardiac end-organ damage, higher aortic stiffness, and stroke volume.

Disparities between determinants of impaired vascular structure and function in young people with primary hypertension: a systematic review

Summary: This study aimed to investigate the early effects and independent determinants of arterial structure and function in children and young people with primary hypertension. The results revealed that elevated blood pressure was associated with higher carotid-femoral pulse wave velocity (cfPWV) and carotid intima-media thickness (cIMT), while body mass index (BMI) showed weaker associations. Improvement in carotid wall cross-sectional area (WCSA) was associated with a decrease in measures of central obesity.

Knowledge gaps and future directions in cognitive functions in children and adolescents with primary arterial hypertension: A systematic review

Summary: Arterial hypertension is associated with poor cognitive outcomes in children and adolescents. The existing literature mainly relies on indirect measures of executive functions and identifies confounders such as obesity and sleep disorders. Future prospective trials should use standardized neuropsychological tests and consider the impact of these confounders on the results.

Prevalence of left ventricular hypertrophy in children and young people with primary hypertension: Meta-analysis and meta-regression

Summary: Left ventricular hypertrophy (LVH) is a main marker of hypertensive organ damage (HMOD) in children and young people (CYP) with primary hypertension(PH). A meta-analysis of CYP with PH was conducted and it was found that the overall prevalence of LVH was 30.5%, with a predominant pattern of eccentric LVH. Increased BMI was identified as the most significant risk factor for LVH in hypertensive youth.

Summary: Primary hypertension is the main form of arterial hypertension in adolescents, closely related to metabolic syndrome and visceral obesity. Metabolic syndrome is also observed in adolescents with primary hypertension. Obesity and metabolic abnormalities can increase the risk of hypertension and target organ damage.

Evolution of isolated systolic hypertension with normal central blood pressure in adolescents-prospective study

Summary: The clinical significance and progression of isolated systolic hypertension with normal central blood pressure, known as spurious hypertension (sHT), in adolescents over a 1-year follow-up period were investigated. The study found that after 1 year of non-pharmacological treatment, 23% of adolescents with sHT developed sustained hypertension, with the main predictor of central systolic blood pressure change being the change in serum uric acid level.

Summary: In hypertensive children, EF1 is preserved or enhanced in those with similar LVMi to normotensive children, but decreases in those with greater LVMi. This decrease in EF1 is associated with prolonged myocardial wall stress and reduced diastolic function.
