Should men undergo MRI before prostate biopsy-CON

Summary: Prostate magnetic resonance imaging (MRI) is commonly used before biopsy to address the issues of overdiagnosis and overtreatment associated with PSA-based screening for prostate cancer (CaP). However, the accuracy of prostate MRI is limited due to technical constraints and inter-observer variability, leading to inadequate negative predictive value (NPV) and relatively low positive predictive value (PPV). The current form of pre-biopsy prostate MRI does not provide a clinically significant benefit and should not be considered routine practice until its accuracy is improved.

A Nationwide Analysis of Risk of Prostate Cancer Diagnosis and Mortality following an Initial Negative Transrectal Ultrasound Biopsy with Long-Term Followup

Summary: This study suggests that men with negative TRUS biopsies have a very low prostate cancer-specific mortality, especially for those with PSA <10 ng/ml. Mortality after re-biopsy is similar to after initial biopsy. Therefore, MRI targeting should only be recommended for men with PSA >10 ng/ml after negative biopsy.

Recommendations include consideration of moderate hypofractionation for intermediate-risk patients and a combination of local treatment with long-term hormonal therapy for cN1 PCa patients.

Effects of Magnetic Resonance Imaging Targeting on Overdiagnosis and Overtreatment of Prostate Cancer

Summary: This study aimed to assess the impact of MRI-targeted biopsy on mortality risk for high-grade prostate cancers identified in men with negative systematic biopsy results. The results showed that a large number of men need to be diagnosed and treated for prostate cancer in order to prevent just one prostate cancer death when targeting MRI-visible tumors during biopsy.
