Postoperative peritoneal adhesion: an update on physiopathology and novel traditional herbal and modern medical therapeutics

Setareh Soltany

Summary: This review focuses on the pathophysiology and underlying signal pathways of postoperative peritoneal adhesion formation, as well as strategies developed using traditional herbal and modern medicines to regulate molecular mechanisms. While various mechanisms have been defined for adhesion formation, conflicting results and animal studies are prominent in the current literature. The use of herbal products as safe and effective agents is recommended for future preclinical and clinical studies, although further research is needed to determine the efficacy of strategies for prevention and treatment of PPA.

Does the timing of protective ileostomy closure post-low anterior resection have an impact on the outcome? A retrospective study

Fozan Sauri et al.

Summary: This study investigated the relationship between the timing of protective ileostomy closure and post-closure complication rates. It found no significant association between the duration of ileostomy closure and the rate of complications. While intestinal obstruction was more common in the late closure group, early closure group had a higher incidence of surgical site infections.

Summary: Every year, the American Cancer Society projects the numbers of new cancer cases and deaths in the United States, with the latest data showing a significant decline in lung cancer mortality, while prostate cancer mortality has plateaued and breast and colorectal cancer mortality have slowed. Improvements in treatment have accelerated progress against lung cancer, leading to a record drop in overall cancer mortality.

Comparative study between ghost ileostomy and defunctioning ileostomy in terms of morbidity and cost-effectiveness in low anterior resection for rectal cancer

Serkan Zenger et al.

Summary: This study compared outcomes of ghost ileostomy (GI) and defunctioning ileostomy (DI) in patients who underwent low anterior resection (LAR) for rectal cancer, finding that GI group had lower rehospitalization rates and total costs. Therefore, in patients with low or medium risk factors, GI may be a safe and cost-effective alternative to DI.

Low anterior resection syndrome-Causes and treatment approaches

Sigmar Stelzner et al.

Summary: The functional sequelae of anterior rectal resection, known as low anterior resection syndrome (LARS), are becoming more predominant with improved oncological prognosis in rectal cancer patients. Approximately 40% of patients are expected to experience major LARS, while about 20% may have minor LARS, with the length of the remaining rectal stump being a key factor. Diagnosis and quantification of LARS are clinically based, and treatment options vary from patient counseling to surgical interventions.

Impact of a defunctioning ileostomy and time to stoma closure on bowel function after low anterior resection for rectal cancer: a systematic review and meta-analysis

I. Vogel et al.

Summary: Impaired bowel function after low anterior resection (LAR) for rectal cancer is a common problem affecting quality of life. A defunctioning ileostomy appears to increase the risk of major LARS, and delaying ileostomy closure may worsen bowel function. Early reversal of ileostomy closure should be considered to mitigate these effects.

Increased risk of complications in smokers undergoing reversal of diverting ileostomy

Benjamin Cribb et al.

Summary: The study found that post-operative complications of diverting ileostomy reversal in patients who underwent rectal cancer surgery were associated with smoking and receipt of adjuvant chemotherapy. Smokers had a higher risk of complications, while patients who received adjuvant chemotherapy had a lower risk of complications.

Anastomotic leak in patients with acute complicated diverticulitis undergoing primary anastomosis: risk factors and the role of diverting loop ileostomy

Rebecca L. Hoffman et al.

Summary: Regardless of the presence of a diverting loop ileostomy, chronic steroid use and smoking are associated with an increased risk of anastomotic leak in patients with acute complicated diverticulitis undergoing colectomy with primary anastomosis. The presence of a diverting loop ileostomy is protective against re-operation.

Summary: This study found that patients in the early closure group had a higher rate of postoperative complications, mostly minor, compared to the late closure group. However, there was no significant difference in the rate of severe complications between the two groups.
