Postoperative peritoneal adhesion: an update on physiopathology and novel traditional herbal and modern medical therapeutics

Summary: This review focuses on the pathophysiology and underlying signal pathways of postoperative peritoneal adhesion formation, as well as strategies developed using traditional herbal and modern medicines to regulate molecular mechanisms. While various mechanisms have been defined for adhesion formation, conflicting results and animal studies are prominent in the current literature. The use of herbal products as safe and effective agents is recommended for future preclinical and clinical studies, although further research is needed to determine the efficacy of strategies for prevention and treatment of PPA.

Summary: This study investigated the relationship between the timing of protective ileostomy closure and post-closure complication rates. It found no significant association between the duration of ileostomy closure and the rate of complications. While intestinal obstruction was more common in the late closure group, early closure group had a higher incidence of surgical site infections.

Approximately 40% of patients are expected to experience major LARS, while about 20% may have minor LARS, with the length of the remaining rectal stump being a key factor. Diagnosis and quantification of LARS are clinically based, and treatment options vary from patient counseling to surgical interventions.

Summary: Impaired bowel function after low anterior resection (LAR) for rectal cancer is a common problem affecting quality of life. A defunctioning ileostomy appears to increase the risk of major LARS, and delaying ileostomy closure may worsen bowel function. Early reversal of ileostomy closure should be considered to mitigate these effects.
