Peripheral Ischemia Imprints Epigenetic Changes in Hematopoietic Stem Cells to Propagate Inflammation and Atherosclerosis

Summary: Peripheral artery disease is associated with increased inflammation and inflammatory cell production, but the underlying mechanisms are not well understood. This study found that patients with peripheral artery disease and mice with hind limb ischemia exhibited elevated leukocyte numbers. Further analysis revealed migration of hematopoietic stem and progenitor cells (HSPCs) from the osteoblastic niche to the vascular niche, along with enhanced proliferation and differentiation. Inhibition of IL-1 and IL-3 receptors resulted in suppressed HSPC proliferation, reduced leukocyte production, and improved atherosclerosis.

The primary treatment goals were increased walking distance or duration, performing specific activities or tasks, and increased walking speed. These treatment goals were associated with patient characteristics and PAD-specific patient-reported outcomes.
