Comparison of Open-Access, Trough-Only Online Calculators Versus Trapezoidal Method for Calculation of Vancomycin Area Under the Curve (AUC)

Summary: This study compared the accuracy of vancomycin AUC calculated by open-access, online, trough-only calculators to AUCs calculated by the trapezoidal method using peak and trough concentrations. The results showed that VancoPK was the closest calculator to the trapezoidal method and had the most consistent dose adjustments. Therefore, VancoPK can be used as an alternative to the trapezoidal method for calculating vancomycin AUC.

Association Between Vancomycin Area Under the Curve and Nephrotoxicity: a single center, retrospective cohort study in a veteran population

Summary: Patients with a vancomycin AUC >=550 were found to have a significantly higher rate of AKI compared with those with an AUC <550. AKI occurred in 42% of patients with AUC =550 compared with 2% of patients with AUC <550. Secondary outcomes were not different between the groups.

The significant institutional cost benefits and potential to improve clinical outcomes make consideration of improved dosing methods for vancomycin highly warranted.

Vancomycin area under the curves estimated with pharmacokinetic equations using trough-only data

Summary: This study compared AUCs estimated from trough-only data to AUCs calculated from peak and trough concentrations. The derived Vd equation provided accurate and precise AUC estimates from trough-only data. The study concluded that accuracy and precision of estimated AUCs depend on the applied population Vd model.
