Laparoscopic distal pancreatectomy in patients with poor physical status

Summary: In a study of 605 patients who underwent Laparoscopic Distal Pancreatectomy (LDP), it was found that patients with ASA grades III-IV were older, mostly male, with more comorbidities, and required more blood transfusion. High-risk patients had significantly higher rates of medical complications, but overall/severe morbidity, surgical complications, and mortality rates were similar. ASA grades III-IV predict medical complications, but do not have a significant impact on surgical complications and severe morbidity of LDP.

However, R0 margins can be obtained with acceptable long-term survival rate in selected patients. Patients over 70 years old and postoperative complications are independent predictors of lower survival.
