Evolving Trends in Pancreatic Cystic Tumors: A 3-Decade Single-Center Experience With 1290 Resections

Jorge Roldan et al.

Summary: The aim of this study was to describe the experience with resected cystic tumors of the pancreas in our institution, focusing on changes in clinical presentation and accuracy of preoperative diagnosis. Results showed an increase in the incidental discovery of pancreatic cystic lesions, leading to more pancreatic resections. Surgical outcomes revealed low operative mortality and relatively high major morbidity, with an improvement in concordance between preoperative and final histopathological diagnosis.

Targeted next-generation sequencing of EUS-guided throughthe-needle-biopsy sampling from pancreatic cystic lesions

Charlotte Vestrup Rift et al.

Summary: This study evaluated the feasibility and diagnostic accuracy of molecular analysis and subtyping of pancreatic cystic lesions (PCLs) using EUS-guided through-the-needle-biopsy (TTNB). The results showed that NGS analysis of TTNB samples has high sensitivity and specificity for diagnosing mucinous cysts and intraductal papillary mucinous neoplasms (IPMNs). The importance of this study is rated 9 out of 10.

Prospective, Multi-Institutional, Real-Time Next-Generation Sequencing of Pancreatic Cyst Fluid Reveals Diverse Genomic Alterations That Improve the Clinical Management of Pancreatic Cysts

Alessandro Paniccia et al.

Summary: This study prospectively evaluated the use of next-generation sequencing (NGS) in the assessment of patients with pancreatic cysts. The results showed that PancreaSeq is a sensitive and specific method for evaluating different types of pancreatic cysts and advanced neoplasia arising from mucinous cysts, and it reveals the diversity of genomic alterations seen in pancreatic cysts and their clinical significance.

Non-functional pancreatic neuroendocrine tumours: ATRX/DAXX and alternative lengthening of telomeres (ALT) are prognostically independent from ARX/PDX1 expression and tumour size

Wenzel M. Hackeng et al.

Summary: Recent studies have shown that ATRX/DAXX and ALT are important prognostic factors for NF-PanNETs, especially in tumors less than or equal to 2.0 cm. These biomarkers are highly specific for pancreatic origin among NET metastases of unknown primary.

KRAS/GNAS-testing by highly sensitive deep targeted next generation sequencing improves the endoscopic ultrasound-guided workup of suspected mucinous neoplasms of the pancreas

Daniel Schmitz et al.

Summary: KRAS/GNAS-testing by deep targeted NGS is an effective method to distinguish mucinous from non-mucinous pancreatic lesions with high sensitivity and specificity. Therefore, it can be used as a single diagnostic test, offering potential improvements in patient management.

Guidelines on management of pancreatic cysts detected in high-risk individuals: An evaluation of the 2017 Fukuoka guidelines and the 2020 International Cancer of the Pancreas Screening (CAPS) consortium statements

Mohamad Dbouk et al.

Summary: In high-risk individuals, the management standards for pancreatic cysts include CAPS and Fukuoka guidelines, with some differences in sensitivity and specificity; These standards perform moderately in predicting the presence of high-grade dysplasia or invasive cancer in cystic lesions; New approaches are needed to guide the surgical management of cystic lesions in high-risk individuals.

Comparative analysis of glucose and carcinoembryonic antigen in the diagnosis of pancreatic mucinous cysts: a systematic review and meta-analysis

Sandra Faias et al.

Summary: Glucose level in pancreatic cystic fluid performs better than carcinoembryonic antigen (CEA) for accurately diagnosing mucinous cysts, with higher sensitivity and fewer false-negative results. Glucose could potentially become a useful first-line test to exclude mucinous cysts, especially in small cysts with limited fluid volume. Larger studies are needed to confirm glucose as the preferred test for mucinous cyst diagnosis.
