Active Surveillance Versus Thyroid Surgery for Differentiated Thyroid Cancer: A Systematic Review

Roger Chou et al.

Summary: This systematic review examines the benefits and harms of active surveillance vs. immediate surgery for low-risk differentiated thyroid cancer (DTC). The results suggest that active surveillance and immediate surgery may be associated with similar mortality, risk of recurrence, and other outcomes in low-risk small DTC patients, but the methodological limitations prevent strong conclusions.

Progression of Low-Risk Papillary Thyroid Microcarcinoma During Active Surveillance: Interim Analysis of a Multicenter Prospective Cohort Study of Active Surveillance on Papillary Thyroid Microcarcinoma in Korea

Eun Kyung Lee et al.

Summary: This multicenter prospective cohort study conducted in Korea suggests that the risk of progression during active surveillance (AS) for low-risk papillary thyroid microcarcinoma (PTMC) is low in the Korean population, but slightly higher than in other populations. Younger age, male sex, and larger tumor size were identified as independent significant factors for disease progression during AS.

Marked Decrease Over Time in Conversion Surgery After Active Surveillance of Low-Risk Papillary Thyroid Microcarcinoma

Takahiro Sasaki et al.

Summary: Active surveillance for low-risk papillary microcarcinoma (PMC) is generally safe, but some patients may undergo conversion surgery due to disease progression, patient preference, physician preference, or other reasons. A study found that patients in the second-half group were less likely to undergo conversion surgery than those in the first-half group, regardless of the reason for surgery. This may be attributed to the accumulation of favorable outcomes with active surveillance, leading to increased confidence among physicians and trust among patients.

Indications and Strategy for Active Surveillance of Adult Low-Risk Papillary Thyroid Microcarcinoma: Consensus Statements from the Japan Association of Endocrine Surgery Task Force on Management for Papillary Thyroid Microcarcinoma

Iwao Sugitani et al.

Summary: The management of patients with low-risk papillary thyroid microcarcinoma (PTMC; T1aN0M0) has become an important clinical issue, with the adoption of active surveillance (AS) to avoid overtreatment gradually increasing worldwide. However, not all PTMCs are suitable for AS, and the Japan Association of Endocrine Surgery has produced consensus statements regarding indications and strategies for AS in adult patients diagnosed with low-risk PTMC. These statements provide guidelines for AS and recommend immediate surgery for certain high-risk features in PTMC patients.

Papillary thyroid microcarcinoma: Is active surveillance always enough?

Rosie Sutherland et al.

Summary: The incidence of papillary thyroid carcinoma is increasing, with a focus on overdiagnosis and disease progression. While most cases follow a chronic disease course, some show more aggressive features. Current guidelines recommend against routine biopsy for nodules <= 10 mm, and active surveillance is recommended for low-risk PTMC.

Tumor Volume Doubling Time in Active Surveillance of Papillary Thyroid Microcarcinoma: A Multicenter Cohort Study in Korea

Meihua Jin et al.

Summary: The study found that TVDT <5 years is a crucial predictor of disease progression in PTMC during active surveillance, indicating new LN metastasis development and tumor enlargement. This early determination of TVDT could assist in tailoring follow-up intensity and determining the need for early surgical intervention.
