Fundamental Pathobiology of Coronary Atherosclerosis and Clinical Implications for Chronic Ischemic Heart Disease Management-The Plaque Hypothesis A Narrative Review

Peter H. Stone et al.

Summary: Recent studies have shown that major adverse cardiac events (MACE) are not only caused by severe coronary obstructions, but also by plaque burden along the entire coronary tree. However, the translation of these findings into clinical management strategies needs further research.

CT or Invasive Coronary Angiography in Stable Chest Pain

Pal Maurovich-Horvat et al.

Summary: This study compared the effectiveness of CT and ICA in diagnosing obstructive CAD. The results showed that among patients with stable chest pain and intermediate pretest probability of CAD, the risk of major adverse cardiovascular events was similar between the CT group and the ICA group. However, the frequency of major procedure-related complications was lower in the CT group.

Would You Recommend a Statin to This Patient for Primary Prevention of Cardiovascular Disease? Grand Rounds Discussion From Beth Israel Deaconess Medical Center

Gerald W. Smetana et al.

Summary: Cardiovascular disease is the leading cause of death in the United States and hypercholesterolemia is a key modifiable risk factor for its prevention. Statins, alongside lifestyle changes, are important for reducing the risk of cardiovascular disease in specific patients. The use of validated risk calculators to estimate the 10-year risk for cardiovascular disease can help identify candidates for statin therapy. This paper discusses the varying guidelines on the use of statins in primary prevention and the potential value of advanced testing to assess eligibility.

Comparative effectiveness of initial computed tomography and invasive coronary angiography in women and men with stable chest pain and suspected coronary artery disease: multicentre randomised trial

Klaus F. Kofoed et al.

Summary: This study compared the effectiveness of computed tomography (CT) and invasive coronary angiography (ICA) in the management of stable chest pain in men and women. The results showed that for patients with an intermediate pretest probability of coronary artery disease, an initial CT scan was associated with fewer major complications in women and a lower frequency of a composite endpoint in men.

Would You Recommend Aspirin to This Patient for Primary Prevention of Atherosclerotic Cardiovascular Disease? Grand Rounds Discussion From Beth Israel Deaconess Medical Center

Risa B. Burns et al.

Summary: Atherosclerotic cardiovascular disease (ASCVD) is still the leading cause of death in the United States, and the role of aspirin in primary prevention remains unclear. Experts suggest that low-dose aspirin may be considered for primary prevention in select adults aged 40 to 70 who are at higher ASCVD risk.

Arterial hypertension and morphologic abnormalities of cardiac chambers: results from the Copenhagen General Population Study

Summary: The study revealed that cardiac chamber enlargement and remodeling were more prevalent in patients with untreated or inadequately treated hypertension. There was a strong link between elevated blood pressure and hypertension-mediated organ damage.

Summary: Coronary CTA is found to be equivalent to ICA in assessing long-term risk in patients with NSTEACS, with the rate of primary endpoint being up to 1.7-fold higher in patients with obstructive CAD compared to those with nonobstructive CAD as defined by both imaging modalities.

Summary: Through conducting a study using CCTA on a random sample of individuals aged 50 to 64, we found that silent coronary atherosclerosis is common in this population, particularly in individuals with higher CAC scores.
