Summary: Clinical toxicology and forensic toxicology are two disciplines of toxicology that focus on the qualitative and quantitative analysis of alcohol, drug abuse, over the counter and prescription drugs or poisons in humans. However, there are still challenges in these fields, such as the transient occurrence of new psychoactive substances, short detection windows of some drugs of abuse, and the difficulty in distinguishing occasional users from chronic/addictive users. Metabolomics, a new research field, has been successfully applied in clinical toxicology and forensic toxicology, using techniques such as nuclear magnetic resonance and high-resolution mass spectrometry. This article provides practical considerations, current applications, and critical discussions on the benefits and limitations of untargeted metabolome profiling in these fields.

Urine drug-testing tampering approaches: Turkish probationers

Hyphenated high-resolution mass spectrometry-the all-in-one device in analytical toxicology?

Summary: This trend article reviews papers with hyphenated high-resolution mass spectrometry (HRMS) approaches applied in analytical toxicology, particularly in clinical and forensic toxicology published since 2016 and referenced in PubMed. The article focuses on the question of whether HRMS has or will become the all-in-one device in these fields as supposed by the increasing number of HRMS presentations at scientific meetings, corresponding original papers, and review articles. Considering the reviewed papers, HRMS is currently the only technique that fulfills the criteria of an all-in-one device for the various applications needed in analytical toxicology.

Advances in drugs of abuse testing

Summary: Drugs of abuse testing is commonly used in clinical and forensic settings, with urine being the preferred specimen type but other emerging types include saliva, sweat, hair, and meconium. Liquid chromatography-tandem mass spectrometry (LC-tandem-MS) has become the new gold standard for confirmatory drug testing due to its ability to analyze a wider range of analytes compared to gas chromatography-mass spectrometry (GC-MS). Technical advancements such as high-resolution mass spectrometry and miniature ambient ionization mass spectrometry have improved the speed and efficiency of drugs of abuse testing.
