Even with high PEEP levels, alveolar overdistension was negligible during mechanical ventilation (MV) in Trendelenburg position with capnoperitoneum.

Summary: This study compared baseline PEEP according to the high PEEP-FiO(2) table and personalized PEEP following an EIT-guided PEEP trial. The results showed that an EIT-guided PEEP trial resulted in a relevant change in PEEP in 63% of patients, suggesting that personalized PEEP can improve respiratory mechanics and oxygenation in patients with ARDS.

The impact of ventilation-perfusion inequality in COVID-19: a computational model

Summary: It is hypothesized that COVID-19 infection may lead to acute respiratory distress syndrome, with severe gas exchange derangements possibly associated with minor pulmonary infiltrates. A computational model suggests that a significant ventilation-perfusion inequality exists in COVID-19 patients, with a likely cause of observed hypoxemia being severe hyperperfusion of poorly ventilated lung regions. High cardiac output and extensive microthrombosis in autopsy further support the hypothesis of pathological perfusion in non/poorly ventilated lung tissue.
