Arterial stiffness preceding metabolic syndrome in 3,862 adolescents: a mediation and temporal causal longitudinal birth cohort study

Temporal longitudinal associations of carotid-femoral pulse wave velocity and carotid intima-media thickness with resting heart rate and inflammation in youth

Summary: This study found temporal longitudinal associations between arterial stiffness in adolescence and the risk of elevated resting heart rate (RHR) and high-sensitivity C-reactive protein (hsCRP). Arterial stiffness appears to precede higher RHR in young adulthood, while elevated hsCRP in adolescence preceded arterial stiffness and carotid intima-media thickness (cIMT) in early adulthood.

Cumulative insulin resistance and hyperglycemia with arterial stiffness and carotid IMT progression in 1,779 adolescents: a 9-yr longitudinal cohort study

Summary: This study investigated the relationship between fasting glucose, insulin, and insulin resistance with arterial stiffness and carotid intima-media thickness in adolescents. The results showed that high insulin levels and insulin resistance had a negative impact on arterial stiffness progression, while high blood glucose was not significantly associated with arterial stiffness or carotid intima-media thickness progression. Therefore, the optimal timing for intervention may be in late adolescence.

Does arterial stiffness mediate or suppress the associations of blood pressure with cardiac structure and function in adolescents?

Summary: There is limited understanding of the role of arterial stiffness in cardiovascular disease risk in the pediatric population. This study found that arterial stiffness may attenuate the adverse effects of increased blood pressure on cardiac structure and function in adolescents. The findings suggest that arterial stiffness could be an intermediate pathway in the relationship between blood pressure and cardiac health.

The results showed that elevated non-HDL-C and LDL-C levels were associated with cIMT progression. However, elevated lipids or dyslipidemia were not associated with cfPWV progression.

Effects of Arterial Stiffness and Carotid Intima-Media Thickness Progression on the Risk of Overweight/Obesity and Elevated Blood Pressure/Hypertension: a Cross-Lagged Cohort Study

Summary: This study found that arterial stiffness, measured by carotid-femoral pulse wave velocity (cfPWV), was associated with the risk of overweight/obesity and elevated blood pressure/hypertension in adolescents. However, carotid intima-media thickness (cIMT) had no associations with these risks. Progression of cfPWV was directly associated with an increase in systolic and diastolic blood pressure, while cIMT progression was associated with adiposity measures and diastolic blood pressure. These findings suggest the importance of preventing arterial stiffness, particularly in the context of obesity and hypertension.

Arterial stiffness precedes hypertension and metabolic risks in youth: a review

Summary: Arterial stiffness is a strong predictor of cardiovascular events and all-cause mortality in middle-aged and old adults. However, it has been limited to being an intermediate marker of atherosclerotic cardiovascular events in adolescents and young adult studies. The lack of normative longitudinal data and repeated gold-standard assessments in the young population has led to a significant knowledge gap in its clinical utility. This review summarizes recent evidence in a large adolescent population, supporting the value of arterial stiffness as a novel risk factor for hypertension, overweight/obesity, and insulin resistance, and provides suggestions for incorporating arterial stiffness measures into routine pediatric and young adult clinical practice.

The impact of progredient vessel and tissue stiffening for the development of metabolic syndrome

Summary: This article presents an alternative hypothesis regarding the pathophysiology of metabolic syndrome, suggesting that increased vessel and organ stiffness plays a key role. Summary: Arterial stiffness is a key marker of risk in hypertension, and can be measured at material or structural levels. Studies have shown that structural stiffness predicts outcomes, with elastic arteries being most sensitive to blood pressure and age. Stiffness is considered an index of vascular aging and affects brain, kidneys, and heart function. Reduction of stiffness through antihypertensive treatments and preventive drugs may have additional benefits beyond lowering blood pressure. Newer anti-inflammatory drugs also have effects on stiffness independent of blood pressure.
