Popping the balloon: Abrupt onset of a spinal CSF leak and spontaneous intracranial hypotension in idiopathic intracranial hypertension, a case report

Gary Sulioti et al.

Summary: This study reports a case of a patient with idiopathic intracranial hypertension who developed a spinal cerebrospinal fluid leak and subsequent spontaneous intracranial hypotension following a pressure-elevating maneuver. The findings raise questions about a potential causative link between elevated CSF pressure and the development of spontaneous intracranial hypotension, and suggest the potential prophylactic benefit of lowering CSF pressure in preventing this condition.

Clinical and imaging outcomes of cerebrospinal fluid-venous fistula embolization

Summary: This study evaluated the clinical outcomes of transvenous embolization in patients with spontaneous intracranial hypotension (SIH). The results showed that using Onyx for the embolization of cerebrospinal fluid-venous fistulas (CSFVFs) is safe and effective, resulting in significant improvement in headache and overall clinical outcomes.

Summary: This study aims to understand the breadth of research on the efficacy of treatment for spontaneous intracranial hypotension (SIH) and identify knowledge gaps for future research. Experimental, observational, and systematic review studies on the efficacy of epidural blood patching and surgery for treating SIH will be included.

Cerebrospinal Fluid-Venous Fistulas: A Systematic Review and Examination of Individual Patient Data

Nathan A. Shlobin et al.

Summary: Spontaneous intracranial hypotension caused by CSF-venous fistula can present as positional headache with thoracic spine being the most common location. Dynamic CT myelogram and digital subtraction myelography are the most accurate diagnostic imaging modalities, while epidural blood patch may only provide temporary relief. Surgical management is preferred for long-term resolution of symptoms.

Clinical Presentation, Investigation Findings, and Treatment Outcomes of Spontaneous Intracranial Hypotension Syndrome A Systematic Review and Meta-analysis

Linda D'Antona et al.

Summary: This study provides a systematic review and meta-analysis of spontaneous intracranial hypotension, highlighting the importance of large volume epidural blood patches in treatment and offering treatment guidance. Data shows that brain magnetic resonance imaging is the most sensitive investigation, and large epidural blood patches have better treatment outcomes.

A systematic review and meta-analysis of factors affecting the outcome of the epidural blood patching in spontaneous intracranial hypotension

Summary: Spontaneous intracranial hypotension is a commonly misdiagnosed condition characterized by non-iatrogenic cerebrospinal fluid hypovolemia secondary to spinal CSF leakage. Epidural blood patching is a commonly used intervention, but research on factors affecting its efficacy is limited.
