The complexity of kidney disease and diagnosing it - cystatin C, selective glomerular hypofiltration syndromes and proteome regulation

Linnea Malmgren et al.

Summary: Estimation of kidney function is commonly done using endogenous GFR-markers creatinine or cystatin C. The recommendation in 2010 to use both markers in parallel led to the identification of selective glomerular hypofiltration syndromes, which are associated with increased mortality and morbidity. These syndromes, characterized by a selective reduction in the glomerular filtration of certain molecules, such as cystatin C, have proteomic changes that promote the development of atherosclerosis and may require new treatment modalities.

The Stockholm CREAtinine Measurements (SCREAM) project: Fostering improvements in chronic kidney disease care

Juan Jesus Carrero et al.

Summary: The SCREAM project aims to quantify potential medication errors, estimate the burden of CKD, and illustrate the value of incorporating kidney function measures into medical decision-making. By utilizing common serum/plasma creatinine testing, SCREAM provides valuable information on the population's health and disease journey in Sweden. The research findings reveal the impact of CKD burden and kidney function on health risks and the risk-benefit ratio of medications.

Impaired selective renal filtration captured by eGFRcysC/eGFRcrea ratio is associated with mortality in a population based cohort of older women

Linnea Malmgren et al.

Summary: It is common for elderly women to have eGFR(cysC)/eGFR(crea) ratio <0.6, which is associated with increased mortality risk.

Association of Intraindividual Difference in Estimated Glomerular Filtration Rate by Creatinine vs Cystatin C and End-stage Kidney Disease and Mortality

Debbie C. Chen et al.

Summary: In patients with CKD, the differences between eGFRcys and eGFRcr are common, and these differences and their changes over time may indicate risks of ESKD and mortality.

A Unifying Approach for GFR Estimation: Recommendations of the NKF-ASN Task Force on Reassessing the Inclusion of Race in Diagnosing Kidney Disease

Cynthia Delgado et al.

Summary: The National Kidney Foundation and the American Society of Nephrology have made recommendations regarding the consideration of race in GFR estimation. They suggest implementing the CKD-EPI creatinine equation without the race variable in all US laboratories and promoting the use of cystatin C for more accurate estimation. They also encourage research on new filtration markers and interventions to eliminate racial disparities. The unified approach should be adopted nationwide.

Association of Intra-individual Differences in Estimated GFR by Creatinine Versus Cystatin C With Incident Heart Failure

Debbie C. Chen et al.

Summary: The differential between eGFR(cys) and eGFR(cr) is associated with the risk of heart failure among patients with chronic kidney disease. Diverging slopes over time between eGFR(cys) and eGFR(cr) are also independently associated with the risk of heart failure.

Concordance between creatinine- and cystatin C-based eGFR in clinical practice

Emil den Bakker et al.

Summary: Through analyzing 1596 simultaneous cystatin C and creatinine measurements in a pediatric nephrological population, it was found that the mean of eGFR(crea) and eGFR(cys) has high accuracy, with the majority of measurements having Delta eGFR below 40%. Measurements with Delta eGFR above 40% were more frequent in patients with neural tube defects, while potential explanations were found for measurements with low agreement.

The difference between cystatin C- and creatinine-based eGFR is associated with adverse cardiovascular outcome in patients with chronic kidney disease

Hyoungnae Kim et al.

Summary: In this study, a large positive difference between eGFR based on creatinine and cystatin C was found to be associated with a higher risk of major adverse cardiovascular events (MACE) and accelerated coronary artery calcification (CAC) progression in patients with chronic kidney disease (CKD). Therefore, careful monitoring of cardiovascular diseases is recommended for patients with a higher eGFR difference.
