Contribution of Reinfection to Annual Rate of Tuberculosis Infection (ARI) and Incidence of Tuberculosis Disease

Summary: Background modeling studies have found that most tuberculosis infections result from reinfection. The annual rate of infection (ARI) may not accurately reflect the risk of infection among previously infected individuals. Using a simulation model, we found that ARI underestimates the true risk of infection, especially in populations with heterogeneous risk. These findings suggest that the true force of tuberculosis infection in a community may be greater than previously appreciated.

Young children are expected to have a lower risk of infection, and individuals may have varying responses to exposure, leading to potentially higher infection risk. Targeting the minority of individuals who have been recently infected may lead to significant reductions in tuberculosis incidence.

Summary: The study found a high detection rate of M tuberculosis complex DNA in CD34-positive PBMCs, which was associated with both HIV-infected and uninfected individuals, with less difference in those with QuantiFERON results. Isoniazid preventive therapy reduced the prevalence of M tuberculosis complex DNA in PBMCs of HIV-infected individuals.

Biomarker-guided tuberculosis preventive therapy (CORTIS): a randomised controlled trial

Resistance to Mycobacterium tuberculosis Infection Among Household Contacts: A Multinational Study

Summary: A multinational study found that at least 10% of household contacts of patients with pulmonary multidrug- or rifampin-resistant tuberculosis are resistant to M. tuberculosis infection, regardless of exposure level. The prevalence of resistance did not significantly differ by sex, age, HIV coinfection, or comorbid conditions. Further research into the genetic or immunologic mechanisms underlying resistance could lead to new therapeutic and vaccine strategies.
