Chronic lymphocytic leukemia (CLL) with Reed-Sternberg-like cells vs Classic Hodgkin lymphoma transformation of CLL: does this distinction matter?

Summary: This retrospective study analyzed the clinicopathologic features of chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) with isolated Hodgkin/Reed-Sternberg cells (CLL-HRS) and overt transformation to classic Hodgkin lymphoma (CLL-HL). The phenotypic features of Reed-Sternberg cells were similar in both groups, but there was variation in CLL/SLL infiltration among the HRS cells. Patients with CLL-HRS had shorter overall survival compared to patients with CLL-HL, and CLL-HRS patients who received Hodgkin-directed therapy had significantly longer overall survival compared to those who received CLL-directed therapy.

Summary: The somatic hypermutation (SHM) status of the clonotypic immunoglobulin heavy variable (IGHV) gene is critical for assessing prognosis and responses to therapy in patients with chronic lymphocytic leukemia (CLL). Immunogenetic analysis in CLL has also revealed distinct subsets of patients based on (quasi)identical B cell receptor immunoglobulin (BcR IG), which display consistent biology, clinical presentation, and outcome. As a cornerstone for accurate risk stratification and clinical decision making, immunogenetic analysis in CLL requires robust methodologies, including next generation sequencing (NGS).

Higher-order connections between stereotyped subsets: implications for improved patient classification in CLL

Summary: The study found that a significant portion of CLL patients belong to stereotyped subsets, with 19 major subsets retaining their relative size and ranking while 10 new ones emerged. Higher-level relationships were evident between subsets, particularly for major stereotyped subsets with unmutated IGHV genes, which were identified to have close relations with other subsets called satellites. Satellite subsets accounted for 3% of the entire cohort, suggesting a novel aspect of repertoire restriction with implications for refined molecular classification of CLL.

Hodgkin lymphoma arising in patients with chronic lymphocytic leukemia: outcomes from a large multi-center collaboration

Ethnic and geographic diversity of chronic lymphocytic leukaemia

Summary: East Asians, Asian Indians, and Amerindians have a significantly lower incidence rate of chronic lymphocytic leukemia compared to persons of European descent, suggesting a genetic rather than environmental basis for this disparity. These populations have different genetic admixtures with archaic hominins, potentially explaining the varying CLL incidences among them, as well as differences in clinical and molecular covariates of CLL. Further research is needed to determine the exact reasons behind these population variances in CLL and other lymphomas and cancers.
