What more can be done? Prioritizing the most promising antenatal interventions to improve birth weight

Summary: Through analysis, we identified six promising antenatal interventions, including provision of multiple micronutrients, low-dose aspirin, high-dose calcium, prophylactic cervical cerclage, psychosocial support for smoking cessation, and other psychosocial support for targeted populations and settings, which are not currently recommended by the World Health Organization (WHO) for the prevention of low birth weight (LBW).

Further research on broader interventions is needed.

A modular systematic review of antenatal interventions to address undernutrition during pregnancy in the prevention of low birth weight

Summary: This systematic review examined the effects of seven antenatal nutritional interventions on the risks of adverse birth outcomes. The findings suggest that supplementation with balanced protein and energy, multiple micronutrients, and lipid-based nutrient supplements can reduce the risk of low birth weight and related outcomes in populations with undernutrition. Further investigation is needed to determine the benefits of omega-3 fatty acids and calcium supplementation, as well as interventions for pregnant women not gaining weight.

Antenatal interventions to address harmful behaviors and psychosocial risk factors in the prevention of low birth weight

Summary: This systematic review examines the effect of eleven antenatal interventions on adverse birth outcomes. The study finds that psychosocial interventions for reducing smoking in pregnancy may reduce the risk of low birth weight, and professionally provided psychosocial support may reduce the risk of preterm birth. However, financial incentives, nicotine replacement therapy, and virtually delivered psychosocial support do not appear to reduce the risk of adverse birth outcomes.

Summary: Malaria is a major cause of adverse pregnancy outcomes in sub-Saharan Africa and resistance to the recommended antimalarial, sulfadoxine-pyrimethamine, is threatening progress. A trial of dihydroartemisinin-piperaquine as an alternative to sulfadoxine-pyrimethamine showed significant reductions in placental malaria and adverse pregnancy outcomes. The results raise questions about the discrepancies with previous trials.
