Does Alzheimer's disease with mesial temporal lobe epilepsy represent a distinct disease subtype?

Summary: Patients with Alzheimer's disease (AD) and mesial temporal lobe epilepsy (MTLE) may represent a distinct subtype of AD. AD and MTLE share histopathological features and involve overlapping neuronal and cortical networks. Hyper-phosphorylated tau (pTau) increases neuronal excitability and provokes seizures, while neuronal excitability increases pTau. The pTau burden in mesial temporal lobe predicts cognitive deficits in AD and MTLE.

Neuropathological Insights into Unexpected Cognitive Decline in Epilepsy

Summary: In this study, extensive neuropathological examinations were conducted on patients showing cognitive decline after epilepsy surgery. The results indicated the presence of secondary neurodegenerative diseases or independent neurodegenerative alterations in addition to typical pathologies. This suggests the need for a multicenter collection of such cases to identify presurgical biomarkers for better surgical decision-making.
