Diabetic macular ischaemia- a new therapeutic target?

Summary: This review discusses the definition and grading issues of diabetic macular ischaemia (DMI), proposing vascular phenotypes of DMI based on relative perfusion deficits. The review highlights mechanistic pathways of DMI, including the role of hypoxia and the complex relation between neurons, glia, and microvasculature. Utilising optical coherence tomography angiography (OCTA) to identify reversible DMI phenotypes and developing new trial endpoints for disease progression are crucial. Natural history studies of DMI are needed to define phenotypes and understand the development of diabetic macular oedema.

Evaluation of vessel density in disorganization of retinal inner layers after resolved diabetic macular edema using optical coherence tomography angiography

Factors based on optical coherence tomography correlated with vision impairment in diabetic patients

Summary: This study aimed to evaluate the relationship between retinal structures and visual acuity in diabetic patients using OCT and OCTA. Factors such as length of DRIL, length of EZ disruption, and FAZ circularity were found to be correlated with poor visual acuity in treatment-naive diabetic eyes. Other factors showed no significant correlation with visual impairment.

Summary: The study assessed the prevalence and relative contribution of avoidable causes of blindness and vision impairment globally from 1990 to 2020, finding that there was no change in the global prevalence of avoidable blindness and vision impairment in adults aged 50 and older. Although the age-standardized prevalence of avoidable blindness decreased, avoidable moderate and severe vision impairment did not change.
