Melatonin Does Not Improve Sleep Quality in a Randomized Placebo-controlled Trial After Primary Total Joint Arthroplasty

Samuel J. Clarkson et al.

Summary: Sleep disturbance is a common issue after total joint arthroplasty (TJA) and can negatively impact quality of life and pain recovery. This prospective study investigated the effects of using exogenous melatonin for 6 weeks post-surgery on sleep quality. The results showed that melatonin did not have a significant effect on improving sleep quality for TJA patients.

Perioperative neurocognitive disorders: A narrative review focusing on diagnosis, prevention, and treatment

Hao Kong et al.

Summary: Perioperative neurocognitive disorders refer to abnormal neurocognitive functions detected during the perioperative periods, and diagnosis can be made using the DSM-5 or other tools. The disorders have multifactorial etiology, including predisposing and precipitating factors, and prevention measures include avoiding potential contributors and implementing interventions, with nonpharmacological treatments being the first-line approach.

Sleep improvement intervention and its effect on patients' sleep on the ward

Marita Ritmala-Castren et al.

Summary: The study aimed to investigate the association between sleep improvement interventions and patients' sleep, finding that interventions targeting sleep quality may be effective, particularly for men. Clinical importance should be placed on investing in nursing interventions to promote patients' sleep, with individualized sleep support being a key component in care plans. Further research is needed to confirm the findings and support more personalized interventions for optimal patient sleep outcomes.

Sleep, Pain, and Cognition: Modifiable Targets for Optimal Perioperative Brain Health

Brian P. O'Gara et al.

Summary: Preventing perioperative neurocognitive disorders is crucial and requires a multicomponent approach due to multiple risk factors present during the perioperative period. Sleep, pain, and cognition play important roles in the risk of perioperative neurocognitive disorders, but the benefits and impacts of interventions targeting these factors are still uncertain. Current best practices involve multimodal analgesia and opioid avoidance to prevent postoperative delirium, although data supporting their efficacy is limited.
