Mechanisms and Drug Therapies of Bioprosthetic Heart Valve Calcification

Shuyu Wen et al.

Summary: Valve replacement is the main therapy for valvular heart disease, with bioprosthetic heart valves (BHV) surpassing mechanical heart valves (MHV) as the leading option due to their excellent properties. However, BHV is prone to structural valve degeneration (SVD) primarily caused by calcification. Understanding the basic mechanisms of calcification is essential for addressing the limited durability of BHV.

Long-Term Stability and Biocompatibility of Pericardial Bioprosthetic Heart Valves

David F. Williams et al.

Summary: The use of bioprosthetic valves in heart valve therapy has evolved over the decades, with a focus on improving production methods to address issues like immune responses and calcification. Recent advancements in cross-linking and decellularization techniques have improved durability, but the need for even longer-lasting and safer treatments for younger patients has become a crucial clinical and social issue. Developing alternative treatment protocols focusing on tissue-protective decellularization and finely-titrated cross-linking sequences show promise for enhancing the longevity of pericardial heart valves.

Summary: Valvular heart disease is a global issue that often requires valve replacement as the corrective treatment. The main limitation with the most common valve replacement option, xenogenic tissue heart valves, is the development of valve deterioration. Research has been conducted to understand the incidence and risk factors associated with valve deterioration, with a focus on the chronic immune-mediated rejection process of the foreign xenogenic-derived tissue and proposed strategies to combat valve degeneration.

Bioengineered percutaneous heart valves for transcatheter aortic valve replacement: a comparative evaluation of decellularised bovine and porcine pericardia

Sugat Ratna Tuladhar et al.

Summary: Glutaraldehyde-treated, surgical bioprosthetic heart valves and percutaneous heart valves (PHVs) often undergo structural degeneration within 10-15 years. Decellularised scaffolds of bovine and porcine pericardia were used to manufacture bioengineered PHVs, which demonstrated satisfactory hydrodynamic performance and resistance to crimping damage without leaflet alteration. The porcine tissue scaffold showed potential for long-term performance as a preferable option compared to bovine tissue in constructing advanced PHV replacements.
