Clinical and genetic spectra of autosomal dominant tubulointerstitial kidney disease

Holly Mabillard et al.

Summary: Autosomal dominant tubulointerstitial kidney disease (ADTKD) is a kidney disease characterized by tubular damage and fibrosis. Mutations in UMOD and MUC1 genes are the most common causes, but other subtypes also exist. Recent international efforts have provided critical insights into the clinical and genetic aspects of ADTKD, as well as advancements in diagnostic approaches and research on cellular pathways.

Summary: Acute renal failure (ARF) with severe loin pain and patchy renal vasoconstriction, known as anaerobic exercise-induced acute kidney injury (AKI), is a rare syndrome. This article reports a case of ALPE in a 16-year-old Japanese boy who experienced vomiting and severe loin pain after Japanese fencing. The diagnosis of ALPE was based on characteristic findings on delayed renal CT scans.

Summary: The Urаt1-UохDKO mice served as a valuable experimental model for investigating RHUC1. The pathogenesis of EIAKI was linked to increased IL-1b levels via NLRP3 inflammasome signaling and Na+-K+-ATPase dysfunction. Additionally, XOIs showed promise as a therapeutic intervention for EIAKI.

Serum uric acid levels and diabetic kidney disease in patients with type 2 diabetes mellitus: A dose-response meta-analysis

Summary: This study assessed the association between serum uric acid (SUA) levels and diabetic kidney disease (DKD) in patients with type 2 diabetes mellitus (T2DM). The findings suggest that higher SUA levels are associated with an increased risk of DKD in T2DM patients. The risk of DKD increases by 24% for each 1 mg/dl increase in SUA. These results highlight the potential of SUA as a predictor for DKD in T2DM patients.

Hypouricemia and Urate Transporters

Naoyuki Otani et al.

Summary: Hypouricemia, a rare disorder, is divided into overexcretion and underproduction types. Renal hypouricemia, the latter type, is the most common in Japan and can increase the risk of exercise-induced acute kidney injury and urolithiasis. It is crucial to conduct research on renal hypouricemia to gain insight into its pathophysiology and develop new treatments for gout and hyperuricemia.

Summary: The study found that serum uric acid levels were significantly higher in Korean men than in women. As age increased, the average uric acid levels and the prevalence of hyperuricemia increased in women, but not in men. Gender and age should be taken into consideration in studies related to uric acid-related diseases.

The Role of Vasopressin V2 Receptor in Drug-Induced Hyponatremia

Sua Kim et al.

Summary: Hyponatremia is a common condition in clinical practice caused by renal water retention. Many medications can induce hyponatremia by affecting the release of arginine vasopressin or potentiating its action in the kidney. A major mechanism of drug-induced hyponatremia is the upregulation of AQP2 from V2R-cAMP-PKA signaling in the absence of AVP stimulation.

Hypothetical Mechanism of Exercise-Induced Acute Kidney Injury Associated with Renal Hypouricemia

Makoto Hosoyamada

Summary: Renal hypouricemia (RHUC) is a hereditary disease characterized by increased renal urate clearance and low levels of uric acid in the blood, often leading to exercise-induced acute kidney injury (EIAKI). The mechanism behind EIAKI in RHUC patients involves disturbances in endothelial cell function during exhaustive exercise, as well as the activation of inflammatory pathways in the kidney post-exercise.

Summary: Uric acid is mainly synthesized in the liver, intestines, and vascular endothelium, with the kidney playing a dominant role in its excretion. Hyperuricemia is significantly associated with the development of the metabolic syndrome, cardiovascular diseases (CVD), and chronic kidney disease (CKD). Lowering uric acid levels may slow the progression of these diseases.
