Evaluation of laboratory predictors for intravenous immunoglobulin resistance and coronary artery aneurysm in Kawasaki Disease before and after therapy

Summary: This study evaluated the clinical and laboratory characteristics of patients with Kawasaki disease (KD) before and after therapy, and found that the B/A ratio before IVIG and CLI and SII after IVIG were risk factors for IVIG resistance. However, the predictive ability of the fluctuations in risk factors before and after therapy was not superior to that of baseline values in predicting IVIG resistance.

Summary: This study investigated the role of prognostic nutritional index (PNI) in predicting intravenous immunoglobulin (IVIG) resistance and cardiovascular complications in Kawasaki disease (KD). The results showed that lower levels of PNI were independent risk factors for initial IVIG resistance and cardiovascular complications. The sampling-time specific PNI cut-off values demonstrated good predictive power for both IVIG resistance and cardiovascular complications.

Risk factors and coronary artery outcomes of coronary artery aneurysms differing in size and emergence time in children with Kawasaki disease

Summary: Coronary artery aneurysm (CAA) is a serious complication of Kawasaki disease (KD) and the leading cause of acquired heart disease in children. This study identified potential risk factors for CAAs and found that low albumin-to-globin (A/G) ratio was associated with the development of mid- to large-sized CAAs in KD patients. Additionally, de novo CAAs after intravenous immunoglobulin (IVIG) treatment were associated with increased risk of progressive CAA and more severe arterial damage. Both mid- to large-sized and de novo CAAs also prolonged the time for coronary artery normalization. These findings emphasize the importance of aggressive risk modifications and close monitoring with frequent echocardiography for children with KD.

Summary: In the Post RAISE study, younger age at fever onset, a larger maximum Z score at baseline, and nonresponsiveness to IVIG plus prednisolone were identified as significant risk factors associated with the development of coronary artery abnormalities. Nonresponders could be identified at diagnosis based on the total bilirubin value. To prevent coronary artery abnormalities, more intensified or adjunctive therapies using other agents should be considered for patients with these risk factors.

Epidemiology and Risk Factors for Giant Coronary Artery Aneurysms Identified After Acute Kawasaki Disease

Summary: The annual prevalence of giant CA aneurysms identified after acute KD did not increase during the study period. Male sex, recurrent KD, IVIG administration within 1-4 days of illness or >=8 days after KD onset, detection of CA dilatations and aneurysms at initial echocardiography, and resistance to IVIG treatment were significantly associated with giant CA aneurysm complications identified after acute KD. Patients with larger CA abnormalities detected at initial echocardiography were independently associated with progression to giant CA aneurysm complications after acute KD regardless of the number of days from onset at treatment initiation.
