Utility of Ultrasound-Guided Attenuation Parameter for Grading Steatosis With Reference to MRI-PDFF in a Large Cohort

Kento Imajo et al.

Summary: This study analyzed the diagnostic accuracy of ultrasound-guided attenuation parameter (UGAP) in grading steatosis with reference to magnetic resonance imaging-based proton density fat fraction (MRI-PDFF). The results showed that UGAP has excellent diagnostic accuracy, good linearity, and can be widely used in clinical trials and patient care.

Complexity of ballooned hepatocyte feature recognition: Defining a training atlas for artificial intelligence-based imaging in NAFLD

Elizabeth M. Brunt et al.

Summary: The study reveals poor agreement among expert hepatopathologists regarding the number of ballooned hepatocytes seen on the same digitized histology images. This has implications for the diagnosis of nonalcoholic steatohepatitis (NASH) and the requirements for demonstrating 'NASH resolution' in support of drug efficacy in clinical trials. Artificial intelligence-based approaches may offer a more reliable way to assess hepatocyte ballooning.

Summary: The study assessed the correlation between iATT and CAP, finding a strong correlation between the two. Additionally, it was found that iATT measurements remained reliable even when the liver stiffness measurement protocol was not followed. These findings have important implications for clinical diagnosis and treatment.

Accuracy of controlled attenuation parameter (CAP) and liver stiffness measurement (LSM) for assessing steatosis and fibrosis in non-alcoholic fatty liver disease: A systematic review and meta-analysis

Yu-Tian Cao et al.

Summary: This meta-analysis evaluated the performance of controlled attenuation parameter (CAP) and liver stiffness measurement (LSM) in assessing steatosis and fibrosis in non-alcoholic fatty liver disease (NAFLD). The results showed that CAP and LSM had good performance in diagnosing mild steatosis, advanced liver fibrosis, and cirrhosis, making them useful for screening NAFLD in high-risk populations. However, the accuracy of CAP in detecting steatosis is reduced in patients with obesity and requires specific diagnostic values. For LSM, the same diagnostic values can be used with appropriate probe selection based on body mass index (BMI) and an automated probe selection tool. Further validation and improvement of CAP and LSM performance are needed for assessing steatosis in patients with obesity, moderate to severe steatosis, and low-grade fibrosis.

Detection of fatty liver using virtual non-contrast dual-energy CT

Pengcheng Peter Zhang et al.

Summary: The purpose of this study was to determine whether liver attenuation measured on dual-energy CT (DECT) virtual non-contrast examinations predicts the presence of fatty liver. The results showed that attenuation measured at VNC CT was moderately correlated with liver fat content and had high specificity for the diagnosis of fatty liver.

Accuracy of Two-Dimensional Shear Wave Elastography and Attenuation Imaging for Evaluation of Patients With Nonalcoholic Steatohepatitis

Dong Ho Lee et al.

Summary: In this study, a multiparametric approach using attenuation imaging and 2D-SWE was evaluated for the detection of steatosis and fibrosis in NAFLD patients. The attenuation coefficient was associated with steatosis grade, liver stiffness with fibrosis stage, and SWDS with lobular inflammation. A risk scoring system based on these parameters showed high accuracy in identifying patients with steatohepatitis.

Tracey G. Simon et al.

Summary: Population-based data in Sweden showed that all histological stages of NAFLD were linked with significantly increased overall mortality, and the risk escalated progressively with worsening histology. Excess mortality was mainly attributed to extrahepatic cancer and cirrhosis, while cardiovascular disease and HCC made modest contributions.

Recent advances in nonalcoholic fatty liver disease metabolomics

Hwi Young Kim

Summary: The clinical spectrum of NAFLD ranges from simple steatosis to NASH, with liver biopsy being the standard for diagnosis. Metabolomics has emerged as a promising diagnostic method to uncover metabolic alterations relevant to NASH progression, with recent research efforts focusing on lipidomics, amino acids, and bile acid metabolomics to discover potential biomarkers.

Summary: The study aimed to test the reproducibility of performing conventional point shear wave elastography (pSWE), auto-pSWE, and ultrasound derived fat fraction (UDFF) in adult livers. Results demonstrated good repeatability and reproducibility in measuring UDFF and SWV in adult livers. Auto-pSWE showed higher reliability, reproducibility, and time efficacy in measuring SWV compared to the conventional pSWE method.

Summary: The transition from NAFLD to MAFLD involves including FLD subjects with metabolic abnormalities in the criteria while excluding those without, leading to a focus on metabolic dysfunction in the MAFLD criteria. Evidence-based clinical practice guidelines for nonalcoholic fatty liver disease/nonalcoholic steatohepatitis 2020

Katsutoshi Tokushige et al.

Summary: NAFLD and NASH have become significant public health issues in both Western countries and Japan, with hepatic fibrosis being a key prognostic factor. Effective screening methods for NAFLD and urgent surveillance for HCC are necessary in Japan, where non-viral liver disease-related HCC cases are on the rise. Cardiovascular disease is the leading cause of death in NAFLD patients, emphasizing the importance of CVD monitoring in clinical practice. Several drug trials are ongoing for NAFLD/NASH therapy, with new treatments like SGLT2 inhibitors and GLP-1 analogues showing promise for future interventions.

Pathological findings of liver steatosis that is difficult to evaluate with ultrasound

Reiichiro Kondo et al.

Summary: Although new quantitative ultrasound methods have been developed for assessing liver fat content, B-mode ultrasound remains the primary method for detecting liver steatosis during medical checkups. However, some pathological cases can lead to false-positive or false-negative results. The degree of fat deposits and the size of fat droplets in the liver can impact the accuracy of diagnosis using B-mode ultrasound. Operators should be aware of cases where liver steatosis is difficult to evaluate with B-mode ultrasound.

Summary: The study evaluated a new CAP method for assessing liver steatosis, showing improved precision ex vivo and in vivo. The new method utilizes continuous ultrasound data during imaging phase, leading to better agreement and correlation with MRI-PDFF.
