Metabolic Syndrome, Nonalcoholic Fatty Liver Disease, and Chronic Hepatitis B: A Narrative Review

Asian perspective on NAFLD-associated HCC

Summary: Recent data shows that non-alcoholic fatty liver disease (NAFLD) has become a significant public health issue in Asia, with a population prevalence of 34%. The rising trend of NAFLD also leads to an increase in NAFLD-associated hepatocellular carcinoma (HCC). This review focuses on the changing epidemiology of HCC in Asia over the past 30 years, highlighting the importance of traditional risk factors and specific genetic polymorphism in East Asia. Additionally, non-obese individuals with NAFLD are commonly observed in Asia.

The Role of Metabolic Factors and Steatosis in Treatment-Naive Patients with Chronic Hepatitis B and Normal Alanine Aminotransferase

Summary: This study aimed to investigate the impact of metabolic syndrome and nonalcoholic fatty liver disease on treatment-naive patients with chronic hepatitis B and normal alanine aminotransferase levels. The results showed that the accumulation of metabolic syndrome components exacerbated hepatic steatosis, and severe nonalcoholic fatty liver disease was independently associated with significant fibrosis. Therefore, the importance of screening for metabolic syndrome and nonalcoholic fatty liver disease in these patients is emphasized.

Large-scale prospective cohort studies may reveal NAFLD subtypes and aid in developing prognostic models for better resource management in combating the NAFLD epidemic.

It is recommended to perform liver biopsy or noninvasive methods for patients with normal ALT and low HBV DNA levels to assess hepatic histopathology.

Summary: Routine blood glucose monitoring at home was associated with HBV infection, which meant people with diabetes may be at high risk of HBV infection. The study showed a correlation between higher frequency of self-monitoring of blood glucose and higher rates of HBsAg positivity, as well as an association between diabetes and HBV infection that requires more attention.

Summary: This study investigated the outcomes of nonalcoholic steatohepatitis (NASH) in chronic hepatitis B (CHB) patients receiving antiviral treatment, revealing that lower BMI and weight reduction positively affected NASH resolution, while virologic factors were not correlated with NASH resolution. The value of weight management in CHB patients during antiviral treatment warrants further evaluation.
