Arrhythmic Events and Mortality in Patients With Cardiogenic Shock on Inotropic Support: Results of the DOREMI Randomized Trial

Summary: Inotropic support is commonly used in the management of cardiogenic shock. This study found that approximately half of the patients on dobutamine or milrinone experienced arrhythmic events, which were associated with adverse clinical outcomes. Five factors may help identify patients at the highest risk of arrhythmic events.

Summary: This study aims to explore the relationship between the severity of coronary artery disease (CAD) and the occurrence of ventricular tachycardia/ventricular fibrillation (VT/VF) in patients with acute myocardial infarction (AMI). The findings suggest that the SYNTAX score (SS) and FSScaFFR are closely associated with the occurrence of VT/VF in AMI patients.

Clinical Outcomes in Patients With Dilated Cardiomyopathy and Ventricular Tachycardia

Summary: Patients with dilated cardiomyopathy (DCM) requiring radiofrequency catheter ablation (RFCA) for ventricular tachycardia (VT) are at high risk. Early VT recurrence and left ventricular ejection fraction (LVEF) ≤ 32% identify patients at very high risk for mortality or heart transplantation (HTx), and screening for mechanical support or HTx should be considered. Late VT recurrence after RFCA does not predict worse outcome.

Prognostic impact of coronary chronic total occlusion on recurrences of ventricular tachyarrhythmias and ICD therapies

Summary: This study investigated the impact of coronary chronic total occlusion (CTO) on the recurrence of ventricular tachyarrhythmias in implantable cardioverter defibrillator (ICD) recipients. The results showed that CTO patients had a higher risk of first recurrence of ventricular tachyarrhythmias, first appropriate ICD therapy, and all-cause mortality at 5 years. Less developed collaterals and a higher J-CTO score were found to be strong predictors of recurrences in CTO patients at 5 years.
