Macrophage-Targeted Sodium Chlorite (NP001) Slows Progression of Amyotrophic Lateral Sclerosis (ALS) through Regulation of Microbial Translocation

Gut- and oral-dysbiosis differentially impact spinal- and bulbar-onset ALS, predicting ALS severity and potentially determining the location of disease onset

Summary: In patients with ALS, different subtypes (sALS and bALS) show different dysbiosis patterns. sALS patients primarily exhibit gut dysbiosis, while bALS patients primarily exhibit oral dysbiosis. For sALS patients, gut dysbiosis is associated with microbial translocation and symptom severity, while for bALS patients, oral dysbiosis is associated with microbial translocation and disease severity. Both subtypes display oral motor deficits, but only in bALS is oral dysbiosis correlated with severity of oral motor deficits.

Phase 2B randomized controlled trial of NP001 in amyotrophic lateral sclerosis: Pre-specified and post hoc analyses

Summary: ALS is a complex disease that may be influenced by inflammation. This study found that NP001, which regulates macrophage activation, could slow down disease progression in ALS patients with higher levels of the inflammatory marker CRP. Although the phase 2B trial did not meet its primary endpoints, post hoc analysis identified a subset of patients aged 40-65 years who showed significantly slower decline in ALSFRS-R scores and vital capacity loss when treated with NP001.
