Cerebrospinal Fluid Biomarkers in Differential Diagnosis of Multiple Sclerosis and Systemic Inflammatory Diseases with Central Nervous System Involvement

CSF CXCL13 and Chitinase 3-like-1 Levels Predict Disease Course in Relapsing Multiple Sclerosis

Summary: This study assessed the concentration of several biomarkers in the cerebrospinal fluid (CSF) of multiple sclerosis (MS) patients and investigated their diagnostic and prognostic significance. The results showed that higher CSF levels of CHI3L1 and CXCL13 were associated with a higher risk of relapse, MRI activity, and overall disease activity in MS patients. These findings suggest that CSF CHI3L1 and CXCL13 levels can serve as good prognostic biomarkers in MS patients and aid in treatment decisions. The study also found differences in CSF BAFF and APRIL levels between different phases of MS, indicating the modulation of B-cells pathways vary in different disease stages.

CSF Kappa Free Light Chains: Cutoff Validation for Diagnosing Multiple Sclerosis

Summary: The study aims to determine and validate a cerebrospinal fluid (CSF) k (KCSF) value comparable to the detection of cerebrospinal fluid-specific oligoclonal bands (OCB) for diagnosing multiple sclerosis (MS). The results show that a KCSF value of 0.1 mg/dL can be used as a valid alternative to OCB testing, providing a standardized quantitative measure with similar sensitivity and specificity, and reducing human error and cost.

An Update on Laboratory-Based Diagnostic Biomarkers for Multiple Sclerosis and Beyond

Summary: Multiple sclerosis (MS) is an immune-mediated central nervous system inflammatory demyelinating disease. Laboratory tests play an important role in the diagnosis of MS. Biomarkers discovered in the past two decades have been incorporated into clinical practice for the diagnosis of MS mimics. This review summarizes the importance of test selection, assay methodology, optimal sample for testing, and diagnostic utility of these biomarkers. Other laboratory testing methods that can aid in the differentiation between MS and biomarker-defined CNS demyelinating diseases are described. The role of biomarkers of disease activity and prognosis is also discussed, along with future perspectives and the potential for discovering additional biomarkers.

=Differentiating central nervous system demyelinating disorders: The role of clinical, laboratory, imaging characteristics and peripheral blood type I interferon activity

Summary: Objective: This study aimed to explore whether distinct clinical, laboratory, and imaging characteristics along with quantitation of peripheral blood type I interferon (IFN) activity could aid in differentiating between multiple sclerosis (MS) and systemic autoimmune diseases (SADs). The results showed that patients with CNS autoimmune disorders have different clinical, imaging, and laboratory characteristics compared to those with MS spectrum disorders. In addition, patients fulfilling SAD criteria showed significantly higher peripheral blood type I IFN activity, indicating it as a potential therapeutic target.

Further multicenter studies are needed to establish the diagnostic sensitivity and specificity of KFLC, using standardized cut-off values and non-linear formulas.
