Controversies exist regarding the definition, genetic background, and clinical aspects of FNMTC. Current guidelines do not recommend screening of family members of patients with differentiated thyroid cancer.

Summary: In pediatric follicular-patterned tumors, DICER1 mutations are the most common genetic alteration, followed by mutations in NRAS, FGFR3, PTEN, and STK11. Other genetic alterations, such as mutations in HRAS, APC, TSHR, CTNNB1, TP53, EIF1AX, FGFR4, GNAS, RET, and SOS1, are rare. DICER1 and RAS mutations are mutually exclusive.
