Global burden of bacterial antimicrobial resistance in 2019: a systematic analysis

Christopher J. L. Murray et al.

Summary: Antimicrobial resistance (AMR) poses a significant threat to global human health, and this study provides the most comprehensive estimates of AMR burden to date. By estimating deaths and disability-adjusted life-years (DALYs) associated with bacterial AMR in 204 countries and territories in 2019, the study highlights the impact of resistance and the leading pathogen-drug combinations contributing to it.

Comparing public attitudes, knowledge, beliefs and behaviours towards antibiotics and antimicrobial resistance in Australia, United Kingdom, and Sweden (2010-2021): A systematic review, meta-analysis, and comparative policy analysis

Olivia Hawkins et al.

Summary: This study compares public knowledge, attitudes, and practices (KAP) regarding antimicrobial resistance (AMR) in Australia, the United Kingdom (UK), and Sweden, as well as the policy approaches taken in each country to address inappropriate antibiotic use. The findings indicate that Sweden has stricter control over community antibiotic use, while the UK and Australia rely more on practitioner and public education. The results also show that respondents in Australia and the UK are more likely to report beliefs and behaviors not aligned with appropriate antibiotic use compared to those in similar studies conducted in Sweden.

Understanding how and why quality circles improve standards of practice, enhance professional development and increase psychological well-being of general practitioners: a realist synthesis

Adrian Rohrbasser et al.

Summary: This study aims to understand how and why participation in quality circles (QCs) can improve the psychological well-being and clinical practice quality of general practitioners (GPs), and to provide evidence-based and practical guidance for maintaining QCs at local and policy levels. The study used a theory-driven mixed method approach and identified that successful QCs require government trust, training, access to educational material and performance data, as well as group dynamics and peer interaction for optimal effectiveness.

The Role of Point-of-Care C-Reactive Protein Testing in Antibiotic Prescribing for Respiratory Tract Infections: A Survey among Swiss General Practitioners

Nahara Anani Martinez-Gonzalez et al.

Summary: Research found that GPs rely heavily on C-reactive protein point-of-care testing (CRP-POCT) in making antibiotic prescribing decisions, showing a preference for using lower CRP cut-offs to guide prescribing for more severe respiratory tract infections. However, patient pressure, diagnostic uncertainty, fear of complications, and lack of understanding of antibiotic resistance are reported as the major barriers to appropriate antibiotic prescribing.

The updated Consolidated Framework for Implementation Research based on user feedback

Laura J. Damschroder et al.

Summary: This study aimed to obtain feedback from experienced CFIR users for updates to the framework. Results showed positive ratings for most evaluation items of CFIR, but users also provided recommendations for improvements. Updates to CFIR include revisions to existing domains and constructs as well as the addition, removal, or relocation of constructs.

Nationwide implementation of online communication skills training to reduce overprescribing of antibiotics: a stepped-wedge cluster randomized trial in general practice

Leon D'Hulster et al.

Summary: This study assessed the effect of implementing online communication skills training for general practitioners in Belgium. The results showed that inviting GPs to complete the online training and providing them with related materials did not reduce antibiotic prescribing. However, GPs who completed the training prescribed 7% fewer antibiotics, especially during winter.

The use of postal audit and feedback among Irish General Practitioners for the self - management of antimicrobial prescribing: a qualitative study

Kevin F. Roche et al.

Summary: This study explores the perceptions of Irish General Practitioners (GPs) towards the national introduction of postal feedback on their antibiotic prescribing behaviours starting from 2019. The findings indicate that while GPs generally welcome the postal audit and feedback, they identify limitations in the quality of the feedback data, perceive a low public health impact of the feedback, and experience difficulties in efficiently processing the audit and feedback information.

Implementing interventions to reduce antibiotic use: a qualitative study in high-prescribing practices

Aleksandra J. Borek et al.

Summary: Through focus group discussions in high-prescribing practices, professionals viewed delayed antibiotic prescriptions and point-of-care C-Reactive Protein testing not as effective clinical tools, but more as social tools for negotiating treatment decisions and educating patients. Ambiguities were identified regarding the timing and impact of these strategies, as well as contextual barriers to their implementation.

Delayed antibiotic prescribing for respiratory tract infections: individual patient data meta-analysis

Beth Stuart et al.

Summary: Delayed antibiotic prescribing is a safe and effective strategy for most patients, including those in higher risk subgroups. Delayed prescribing was associated with similar symptom duration as no antibiotic prescribing and is unlikely to lead to poorer symptom control than immediate antibiotic prescribing. Delayed prescribing could reduce reconsultation rates and is unlikely to be associated with an increase in symptoms or illness duration, except in young children.

Tiago Costa et al.

Summary: The study characterizes and evaluates the unit dose system for oral antibiotics dispensing at HDES-PSU and extrapolates the results at the Portuguese nationwide level. The results suggest that a unit dose dispensing system can help solve the socioeconomic problems raised by leftovers of oral antibiotics dispensing practices.

Optimising antimicrobial use in humans-review of current evidence and an interdisciplinary consensus on key priorities for research

Esmita Charani et al.

Summary: The focus of the 2021 G7 meeting is on addressing the silent pandemic of antimicrobial resistance (AMR) and achieving antimicrobial security through a balance between new drug development and the preservation of existing agents. Research priorities for optimizing antimicrobial use in humans include policy and strategic planning, medicines management, technology optimization, and context, culture, and behaviors. Sustainable progress depends on developing appropriate interventions, better use of data, and supporting technological innovation.

Improving antibiotic initiation and duration prescribing among nursing home physicians using an audit and feedback intervention: a theory-informed qualitative analysis

Celia Laur et al.

Summary: This study explored antibiotic prescribing practices of nursing home physicians in Ontario and examined the impact of personalized audit and feedback reports on their prescribing behaviors. Professional role and identity were found to significantly influence antibiotic prescribing behaviors, with key roles including being an 'Appropriate prescriber', an 'Educator', and a 'Change driver'. The study suggests that appealing to prescribers' perceived roles could be a unique opportunity to encourage desired changes in antibiotic prescribing.
