Preliminary study of automated oxygen titration at birth for preterm infants

Sanoj K. M. Ali et al.

Summary: This study aimed to investigate the feasibility of using automated oxygen control in the delivery room for preterm infants. The results showed that there was no significant difference in the time to achieve the target oxygen saturation between automated control and manual control, but automated control significantly reduced the time spent in potential hyperoxaemia. In addition, automated control required frequent adjustments to FiO(2), while manual control did not. In terms of oxygen exposure, the automated control group had a higher exposure. Therefore, the use of automated oxygen titration for delivery room management of preterm infants is feasible but requires further evaluation.

Summary: This study aims to develop comprehensive clinical practice guidelines for the assessment and management of sedation, pain, and delirium in critically ill infants and children. The taskforce issued 44 recommendations and five good practice statements, emphasizing the importance of pain monitoring, protocolized sedation and analgesia, and nonpharmacologic interventions for enhancing patient comfort and comprehensive care provision.

Automated control of oxygen titration in preterm infants on non-invasive respiratory support

Peter A. Dargaville et al.

Summary: The study showed that the VDL1.1 algorithm was effective in targeting SpO2 in preterm infants on non-invasive respiratory support, increasing the time spent in the target SpO2 range and reducing exposure to extremes of SpO2. The algorithm functioned effectively in every infant, resulting in greater TR time and fewer episodes of hypoxemia and hyperoxemia.

Summary: This study aimed to evaluate the association between arterial hyperoxia and clinical outcomes in critically ill children using different definitions of hyperoxia. A total of 16 studies involving 27,555 patients were included. The meta-analysis showed a positive association between hyperoxia and mortality. The findings emphasize the need for further prospective observational studies.

Summary: The study compared the effects of two different automated oxygen control devices on target range (TR) time and occurrence of hypoxaemic and hyperoxaemic episodes. The OxyGenie control algorithm was found to be more effective in maintaining oxygen saturation within the target range and preventing hyperoxaemia.

Summary: This study aims to investigate the impact of closed-loop automated oxygen control systems compared to manual oxygen control on the duration of mechanical ventilation in preterm infants. The outcomes include achievement of oxygen saturation targets, time spent in hypoxia or hyperoxia, number of manual adjustments, among others. The study will be conducted following informed parental consent and has been approved by the research ethics committee.

Predictive Intelligent Control of Oxygenation (PRICO) in preterm infants on high flow nasal cannula support: a randomised cross-over study

Koen P. Dijkman et al.

Summary: In preterm infants on high flow nasal cannula support, automated FiO2-control by PRICO is superior to routine manual control in maintaining SpO2 within target range, reducing time spent below target range, above target range, and in severe hypoxia, as well as decreasing manual adjustments. Further validation studies with a higher sample frequency and different ventilation modes are needed.
