Summary: Our study comprehensively assessed the composition of CD8(+) T cell memory subsets and their association with inflammation-related molecules in TB-HIV coinfected patients initiating ART. We found that TB-IRIS individuals have higher frequencies of antigen-experienced CD8(+) T cells, and these cells positively correlate with mycobacterial smear grade, indicating a potential role of CD8(+) T cells in TB-IRIS development. We also observed distinct patterns of correlation between CD8(+) T cell memory subsets and inflammatory cytokines, suggesting different dynamics of CD8(+) T cell memory subsets reconstitution in TB-IRIS patients.

In HIV-1 infected individuals, SNPs in the IL-1 beta gene were associated with a higher risk of TB onset, while variants in the CARD8 gene were associated with a lower risk.

Summary: The study found that systemic inflammatory profiles in people with severe immunosuppression can predict the development of immune reconstitution inflammatory syndrome (IRIS). By developing a composite score incorporating various biomarkers, including interleukin-6, IL-10, IL-27, sCD14, interferon-gamma, tumor necrosis factor-alpha, hyaluronic acid, D-dimer, body mass index, and hemoglobin, accurate predictions of IRIS occurrence and death risk can be made.

TB-IRIS patients exhibit lymphopenia before ART, increased T lymphocyte activation, proliferation, and cytotoxicity, and higher levels of activated and proliferative CD4(+) T cells post-ART. Compensatory mechanisms may exist for Mtb-infected cell elimination. The combination of CD4(+) and CD8(+) T cell markers can better diagnose TB-IRIS during IRIS events.
