Group B Streptococcal Neonatal Meningitis

Teresa Tavares et al.

Summary: Neonatal bacterial meningitis caused by Group B Streptococcus (GBS) is a severe disease associated with high mortality and neurological disability. Maternal colonization with GBS is the main risk factor for neonatal invasive disease. Despite preventive measures and improved neonatal care, the incidence and morbidity of GBS meningitis have not decreased. Many survivors suffer from complex neurological or neuropsychiatric sequelae.

Epidemiology of group B streptococcal disease in infants younger than 1 year in Japan: a nationwide surveillance study 2016-2020

Meiwa Shibata et al.

Summary: The study aimed to define the burden and clinical features of invasive group B streptococcus (GBS) disease in infants younger than 1 year in Japan, explore the transmission route of late-onset disease (LOD), and identify risk factors associated with recurrent GBS disease. The study found that GBS disease still poses a considerable burden with significant mortality and morbidity in Japan. Prematurity and antepartum maternal GBS colonization were significantly associated with the risk of recurrence. The findings provide important information for the development of better strategies for the prevention of GBS disease, including maternal vaccination.

Molecular characterization of pathogenic group B streptococcus from a tertiary hospital in Shanxi, China: High incidence of sequence type 10 strains in infants/pregnant women

Li Zhang et al.

Summary: The study reveals that Group B streptococcus is prevalent in invasive disease in infants and pregnant women, with the hypervirulent clone sequence type 10 being the most common. The PI-1 thorn PI-2a sub-lineages are of significance in infant infections.

Summary: Detection of meningitis in young infants remains challenging, with WHO-recommended signs showing limited sensitivity and specificity. History of fever improves sensitivity but reduces specificity, highlighting the need for careful evaluation and appropriate diagnostic tools for management.

Group B Streptococcal Disease in England (1998-2017): A Population-based Observational Study

Seilesh Kadambari et al.

Summary: This study assessed the burden of GBS disease and mortality in young infants in England, finding that there was no significant change in the annual incidence of GBS disease over the years, but a significant decline in GBS-attributable mortality. Infants with GBS had higher rates of visual impairment, cerebral palsy, hydrocephalus, and necrotizing enterocolitis compared to those without GBS.

Prevalence of Capsular Serotype, Pilus Island Distribution, and Antibiotic Resistance in Pediatric and Adult Invasive Group B Streptococcus Isolates: Data From a Nationwide Prospective Surveillance Study in Germany

Florens Lohrmann et al.

Summary: GBS infection in neonates poses significant life-threatening risks. Vaccination of pregnant women is a potential effective approach. The study found age-dependent differences in capsular serotype and pilus type distribution in neonatal and adult isolates, as well as variations in antibiotic resistance patterns.

The Epidemiology of Meningitis in Infants under 90 Days of Age in a Large Pediatric Hospital

Timothy A. Erickson et al.

Summary: A retrospective study at Texas Children's Hospital between 2010 and 2017 found that viral infections were the main cause of meningitis in infants, while bacterial infections were mainly caused by Group B Streptococcus and Gram-negative rods. Premature birth and Black race were significant risk factors for bacterial meningitis.

Summary: Streptococcus agalactiae, especially the Ib/CC12 strain, poses a significant threat to young infants with its high mortality rate and multidrug resistance. Prevention and control measures for this infection should be emphasized.

Complicated Streptococcus agalactiae Sepsis with/without Meningitis in Young Infants and Newborns: The Clinical and Molecular Characteristics and Outcomes

Chih Lin et al.

Summary: The study revealed that 36.7% of neonatal invasive GBS diseases were complicated with sepsis, with some cases involving meningitis. The research also indicated that antimicrobial resistance among invasive GBS isolates has been increasing, and thrombocytopenia and respiratory failure are independently associated with final adverse outcomes.

Invasive Group B Streptococcal Disease in Neonates and Infants, Italy, Years 2015-2019

Roberta Creti et al.

Summary: Invasive infections by group B streptococci (iGBS) are the leading cause of sepsis and meningitis in neonates and infants. Research in Italy from 2015 to 2019 found that the main clinical manifestations were sepsis, meningitis, bacteremia, and septic shock, with GBS serotype III being predominant in early-onset disease (EOD) and late-onset disease (LOD). Resistance to clindamycin was significant, with MDR-CC17 causing healthcare-associated infections. Monitoring of iGBS characteristics is crucial for prevention strategies and GBS vaccine development.

Genomic Characterization of Serotype III/ST-17 Group B Streptococcus Strains with Antimicrobial Resistance Using Whole Genome Sequencing

Jen-Fu Hsu et al.

Summary: The antibiotic resistance rates were found to be higher in type III/ST-17 Streptococcus agalactiae strains, with loss of PI-1 and ISSag5 and acquisition of ICESag37 playing a key role in the resistance mechanism. This highlights the urgent need for continuous surveillance of GBS infections with the emergence of a hypervirulent strain with antibiotic resistance.

Emergence of Invasive Serotype Ib Sequence Type 10 Group B Streptococcus Disease in Chinese Infants Is Driven by a Tetracycline-Sensitive Clone

Li Zhang et al.

Summary: Group B streptococcus (GBS) is a leading cause of serious infections in infants. The ST10 strain, particularly type Ib/ST10, is responsible for a significant proportion of infant invasive GBS infections in China, leading to serious clinical complications. Continued monitoring and research on this microbiological strain is crucial for understanding and addressing the ongoing emergence of multi-drug resistant GBS isolates.

Mortality, neurodevelopmental impairments, and economic outcomes after invasive group B streptococcal disease in early infancy in Denmark and the Netherlands: a national matched cohort study

Erzsebet Horvath-Puho et al.

Summary: This study evaluated the long-term effects of infant invasive Group B Streptococcus (GBS) disease on mortality, neurodevelopmental impairments, and economic outcomes up to adolescence in Denmark and the Netherlands. The findings showed that meningitis was associated with increased mortality at age 5, any iGBS disease increased the risk of NDI at age 10, and children with a history of iGBS disease required more frequent outpatient clinic visits and hospital admissions by age 5.
