Peripheral nerve morphology and intraneural blood flow in chronic kidney disease with and without diabetes

Tushar Issar et al.

Summary: Sonographic alterations of peripheral nerves in patients with pre-dialytic kidney disease, particularly in those with diabetic kidney disease, were investigated in this study. The researchers found that diabetic kidney disease patients had more severe neuropathy, larger tibial nerve cross-sectional area, and greater median nerve blood flow compared to chronic kidney disease patients. Blood flow was found to be correlated with serum potassium levels. These findings highlight the utility of ultrasound in assessing nerve pathology in kidney disease patients.

Association between the uremic toxins indoxyl-sulfate and p-cresyl-sulfate with sarcopenia and malnutrition in elderly patients with advanced chronic kidney disease

Lara Caldiroli et al.

Summary: In a cross-sectional study of 93 patients with chronic kidney disease, it was found that indoxyl sulfate (IS) and p-cresyl sulfate (PCs) were not associated with sarcopenia, although they were linked to certain inflammatory pathways. Particularly, p-cresyl sulfate was positively associated with the Protein Energy Wasting syndrome (PEW).

Pathogenesis, diagnosis and clinical management of diabetic sensorimotor peripheral neuropathy

Summary: This article discusses the latest advances in the mechanisms of DSPN and painful DSPN, as well as the emerging diagnostics and treatments. While our understanding of the disease has come from animal models, effective therapies in preclinical models have not translated into clinical trials. Management of painful DSPN remains suboptimal, but a mechanism-based approach may offer enhanced pain relief.
