The endoscopic transcortical-transventricular approach is safe and convenient for managing cystic lesions in the third ventricle, including craniopharyngiomas. This minimally invasive approach is increasingly recognized as useful in treating craniopharyngiomas extending into the third ventricle.
The endoscopic transcortical-transventricular approach is safe and convenient in managing cystic lesions, including craniopharyngiomas in the third ventricle. These kinds of tumors are commonly approached through transcranial or transnasal routes. In certain cases, endoscopic transcortical-transventricular approach is indicated if the tumor is localized in the third ventricle with minimal parts in the suprasellar region. Because this approach is minimally invasive, its usefulness and usage have been increasingly recognized in treating craniopharyngiomas extending in the thirdventricle.1-3Other cyst draining techniques, such as the stereotactic approach, could face technical challenges when encountering a multiloculated tumor, where all cysts must be fenestrated and excised. We present a video illustration describing the steps and technical note in the treatment of multicystic craniopharyngioma obstructing the anterior and posterior parts of the third ventricle. The patient was consented to the procedure. This study was approved by the local ethics board (BB155/17).
