A How to Guide: Clinical Population Test Development and Authorization of MALDI-ToF Mass Spectrometry-Based Screening Tests for Viral Infections

Ray K. Iles et al.

Summary: Applying MALDI-ToF mass spectrometry as a clinical diagnostic test for viruses is fundamentally different from its application for bacteria, fungi, and other micro-organisms due to variations in systems biology, viral protein characteristics, and quantification methods. Overcoming analytical challenges in developing clinical MALDI-ToF mass spectrometry tests for virus, particularly human pathogenic enveloped viruses, includes sample enrichment, virus envelope disruption, optimal matrix formulation, optimal MALDI ToF MS performance, and optimal spectral data processing/bioinformatics. The instrument operating settings need to be optimized to match the nature of viral-specific proteins, and the procedures for the technique must meet legal and operational requirements for high-throughput clinical testing. Regulatory authorities often require application of CLIA laboratory developed test (LDT) criteria for clinical MALDI ToF mass spectrometry due to its novelty and lack of standardized comparisons with authorized technology tests.
