This case report presents a successful management of a 29-year-old female SCAD patient with primary percutaneous coronary intervention.

Mortality in spontaneous coronary artery dissection: A systematic review and meta-analysis

Summary: This systematic review and meta-analysis revealed that SCAD is associated with a favorable survival outcome with a mortality rate of 1% over an average follow-up period of 33 months. Male sex and smoking were identified as factors associated with an increased risk of mortality.

Percutaneous coronary intervention with drug-eluting stents versus coronary artery bypass grafting in left main coronary artery disease: an individual patient data meta-analysis

Summary: A comparison of long-term outcomes between PCI with drug-eluting stents and CABG in patients with left main coronary artery disease showed no significant difference in 5-year all-cause mortality. However, differences were observed in rates of myocardial infarction, stroke, and repeat revascularisation, with higher risks associated with PCI. A Bayesian approach suggested a slight advantage favoring CABG in terms of mortality.

Summary: This study compared outcomes of PCI versus conservative therapy in SCAD patients and found that the PCI group had significantly higher rates of major adverse cardiovascular events during hospitalization and at long-term follow-up. Therefore, it is recommended that conservative therapy be the first-line management for SCAD patients unless high-risk features are present.
