Primary Nonselective Laryngeal Reinnervation in Iatrogenic Acute Recurrent Laryngeal Nerve Injury: Case Series and Literature Review

Summary: This study presented the multidimensional voice parameters of 3 patients undergoing primary nonselective laryngeal reinnervation (LR) with concurrent transoral injection laryngoplasty (IL) during thyroidectomy. The results showed that the voices were within normal range within 3 months postintervention, and the laryngeal electromyography depicted evidence of successful reinnervation. This is the first demonstration of the voice outcomes of this combination treatment.

Swallowing disorders after thyroidectomy: a systematic review and meta-analysis

Summary: This systematic review investigates the prevalence of dysphagia after thyroidectomy and finds that swallowing impairment significantly increases shortly after surgery but returns to pre-operative levels 2-3 months later. A small proportion of patients still report dysphagia even 1 year after surgery. There is no significant difference between open and endoscopic thyroid surgery at 2-3 months post-surgery.

Crumley's Classification of Laryngeal Synkinesis: A Comparison of Laryngoscopy and Electromyography

Summary: The study aimed to evaluate the accuracy of Crumley classification in describing laryngeal synkinesis and comparing predicted with actual laryngeal electromyography (LEMG) results. The results showed that local laryngologists and Crumley were able to classify most cases into one of the synkinesis types, but there were discrepancies between predicted and actual LEMG data.

The value of intraoperative nerve monitoring against recurrent laryngeal nerve injury in thyroid reoperations

Summary: Reoperative thyroidectomy is challenging due to the higher incidence of recurrent laryngeal nerve (RLN) injury. In this study, intraoperative nerve monitoring (IONM) was found to improve RLN identification rate and reduce surgeon-related RLN injury rate, but did not significantly reduce tumor-related or scar-related RLN injury rate. Larger prospective studies may be needed to further evaluate the role of IONM in thyroid reoperations.

Techniques include micro-suturing and the use of fibrin glue. It is important to repair the RLN promptly to maintain vocal cord tone, achieve better voice recovery, and prevent aspiration.
