Myocardial injury in noncardiac surgery

Summary: Myocardial injury, characterized by elevated cardiac troponin (cTn) levels without ischemic symptoms, is associated with increased postoperative mortality after noncardiac surgery. The diagnostic criteria for myocardial injury after noncardiac surgery (MINS) include elevated cTn levels within 30 days of surgery with no evidence of non-ischemic causes. Most cases of MINS are asymptomatic and result from an imbalance in oxygen supply and demand. Risk factors for MINS include comorbidities, anemia, glucose levels, and intraoperative blood pressure. Modifiable factors may help prevent MINS, but further research is needed. Recent guidelines recommend routine monitoring of cTn levels during the first 48 hours post-operation in high-risk patients, as MINS often occurs within the first 3 days after surgery without symptoms. Cardiovascular drugs, such as aspirin, antihypertensives, and statins, have shown beneficial effects in MINS patients, and direct oral anticoagulants have been found to reduce MINS-associated mortality in a randomized controlled trial. Preoperative detection of myocardial injury has also been associated with postoperative mortality, but more studies are required.
