Essential Principles and Recent Progress in the Development of TSPO PET Ligands for Neuroinflammation Imaging

Summary: The translocator protein 18kDa (TSPO) is a protein expressed in the outer mitochondrial membrane that is involved in cholesterol transport and steroidogenesis. It is upregulated in response to brain injury and inflammation, making it a potential biomarker for assessing gliosis in brain lesions. Several radioligands for PET imaging targeting TSPO have been developed, but they have limitations such as low sensitivity and dependence on genetic variations. Efforts are currently focused on developing improved radioligands that combine the advantages of second-generation ligands with the ability to overcome binding affinity differences across the population.

Evaluation of [18F]F-DPA PET for Detecting Microglial Activation in the Spinal Cord of a Rat Model of Neuropathic Pain

Summary: In this study, the feasibility and sensitivity of [F-18]F-DPA PET imaging for detecting spinal cord microglial activation after partial sciatic nerve ligation (PSNL) in rats were evaluated. The results showed that [F-18]F-DPA aided visualization of the spinal cord inflammation site in ex vivo autoradiography and was superior to [C-11]PK11195. However, in vivo [F-18]F-DPA PET did not allow for visualization of tracer accumulation.

Symptom-based pharmacotherapy for neuropathic pain related to spinal disorders: results from a patient-based assessment

Summary: Existing guidelines recommend an updated therapeutic algorithm for chronic neuropathic pain, but individualized pharmacotherapeutic management based on pain phenotypes may achieve higher efficacy. This retrospective study evaluated the efficacy of medications based on neuropathic pain phenotypes and proposed symptom-based pharmacotherapy. The findings could contribute to better strategies for symptom-based pharmacotherapeutic management.

New challengers insensitive to TSPO human polymorphism are under evaluation.
