Assessment and treatment of osteoporosis and fractures in type 2 diabetes

Angela Sheu et al.

Summary: There is substantial evidence linking type 2 diabetes to skeletal fragility, despite preserved bone mineral density. The post-fracture outcomes in diabetic patients are worse, raising questions about the ideal assessment and treatment of bone health. Current fracture risk calculators are not effective in predicting fracture risk in these patients, and there is a lack of dedicated randomized controlled trials to identify optimal management for patients with type 2 diabetes.

Bone Density and Structure in Overweight Men With and Without Diabetes

Jakob Starup-Linde et al.

Summary: This study compared bone structure, bone density, and bone turnover markers between non-diabetic overweight men with MetS and overweight men with T1D or T2D. The results showed lower bone turnover in men with T2D, but no clinically significant differences in bone density or structure.

Summary: Bone fragility is a common complication in subjects with type 2 diabetes mellitus. Traditional techniques for evaluating bone fragility do not perform well in this population, but new technologies and biomarkers are emerging as potential solutions.

In Men With Obesity, T2DM Is Associated With Poor Trabecular Microarchitecture and Bone Strength and Low Bone Turnover

Francesca Vigevano et al.

Summary: In men, the combination of obesity and T2D is associated with reduced bone turnover and poorer trabecular bone microarchitecture and bone strength compared to those who are obese but without T2D, suggesting worse bone disease.

Do proximal hip geometry, trabecular microarchitecture, and prevalent vertebral fractures differ in postmenopausal women with type 2 diabetes mellitus? A cross-sectional study from a teaching hospital in southern India

J. Paul et al.

Summary: This study from southern India found that postmenopausal women with diabetes had significantly impaired trabecular microarchitecture and proximal hip geometry compared to age and BMI matched non-diabetic controls, despite no significant difference in bone mineral density at the femoral neck and hip. The study also showed that one-third of women with type 2 diabetes had prevalent vertebral fractures, indicating the limitations of using bone mineral density assessment alone in reflecting bone health in diabetic individuals.

Lower trabecular bone score in type 2 diabetes mellitus: A role for fat mass and insulin resistance beyond hyperglycaemia

Maria Hayon-Ponce et al.

Summary: In this study, it was found that T2DM patients have significantly lower trabecular bone score (TBS) despite higher bone mineral density (BMD) in the lumbar spine. TBS was negatively correlated with BMI, waist circumference, and sex hormone-binding globulin, and positively correlated with BMD. Relative fat mass (RFM) was identified as an important factor influencing TBS in T2DM patients.

Summary: Increased fracture risk in patients with type 2 diabetes mellitus may be due to impairments in bone material properties and increases in cortical porosity, highlighting potential areas for therapeutic intervention.
