Within-Host Genotypic and Phenotypic Diversity of Contemporaneous Carbapenem-Resistant Klebsiella pneumoniae from Blood Cultures of Patients with Bacteremia

Summary: This study is the first genome-wide analysis of within-host diversity of Klebsiella pneumoniae strains recovered from individual patients with bloodstream infections (BSIs). Our finding challenges the single-organism hypothesis and suggests that at least some BSIs are caused by mixed bacterial populations that are unrecognized by the clinical laboratory. The data support a model of pathogenesis in which pressures in vivo select for strain variants with particular antibiotic resistance or virulence attributes and raise questions about laboratory protocols and treatment decisions directed against single strains.

Molecular epidemiology and mechanism of Klebsiella pneumoniae resistance to ertapenem but not to other carbapenems in China

Summary: This study investigated the molecular epidemiology and resistance mechanism of K. pneumoniae strains that are susceptible to meropenem and imipenem but resistant only to ertapenem. It was found that ramR mutation may be related to ertapenem resistance, and mutations in ramR cause efflux pump inhibition and over-expression of outer membrane protein OmpK35 in some strains.

Molecular Epidemiology, Clinical Characteristics and Risk Factors for Bloodstream Infection of Multidrug-Resistant Klebsiella pneumoniae Infections in Pediatric Patients from Tianjin, China

Summary: This study investigated the molecular epidemiology and drug resistance mechanism of pediatric Multidrug-Resistant Klebsiella pneumoniae infections. The results showed that ST11 was the prevalent clone, and ST11 CRKP strains carrying blaNDM-5 and blaKPC-2 genes were resistant to most tested antibiotics. CRKP infections mainly occurred in young immunocompromised patients, with chemotherapy for hematological malignancies being an independent risk factor for bloodstream infections.

CR-hvKP strains, integrating both phenotypes, have become increasingly prevalent globally, primarily in Asia. The mechanisms for the emergence of CR-hvKP can be summarised by three patterns, and continued surveillance of their dissemination should be highly prioritised.
